    MEMORANDUM

    FOR : The Acting Chief, RSTU1

    SUBJECT : Incident Report re Hospitalization of One BISOC Trainee

    DATE : April 14, 2023

    1. This pertains to an incident that needs medical intervention involving one (1)
    BISOC Trainee that transpired at BISOC Training Ground, Sitio Bilogan, Barangay
    Paagan, Santol, La Union.

    2. In this regard, please be informed that at about 02:30 PM of April 14, 2023,
    the duty Assistant Instructor, PSSg Getrone Magne C Malaga, brought one BISOC
    Trainee in the person of Pat Raffy Cortez Usana, 28 years of age, married, a resident of
    Barangay Patong, Magsingal, Ilocos Sur at Bethany Hospital and Medical Center,
    located at Widdoes Street, City of San Fernando, La Union for medical check up due to
    difficulty in breathing.

    3. Initial inquiry revealed that on or about 09:00 AM of April 12, 2023, Pat Usana
    was brought to Santol RHU because his left eye was accidentally injured, hence he was
    administered with an anti-tetanus toxoid vaccine. At around 05:30 PM of April 13, 2023,
    BISOC Trainees to include Pat Usana performed a road run along Sitio Bilogan farm-to-
    market road wherein Pat Usana fell on the ground because of muscle cramps on his
    right foot. After treatment of his cramps, he felt dizzy and suddenly dropped
    unconscious. He was rushed at Balaoan District Hospital for immediate medical
    attention and rested in the said hospital for almost five (5) hours. After which, he was
    released and advised to have bed rest and consumed the prescribed IV fluid. However,
    in the afternoon of April 14, 2023, Pat Usana complained of weakness and difficulty in
    breathing thus, he was brought at Bethany Hospital for immediate medical attention.

    4. The initial diagnosis of Pat. Usana’s attending physician, Dr. Valtazar Rivera,
    was Electrolyte Imbalance (potassium deficiency) that needs complete bed rest.
