    D, a 9-year-old unimmunized girl, presented with diphtheria symptoms similar to her infected friend. Bacterial culture and staining confirmed Corynebacterium diphtheriae infection. The bacteria releases diphtheria toxin fragments A and B. Fragment B binds to host cell receptors, triggering toxin entry and endocytosis. Within endosomes, fragment A is released into host cell cytoplasm where it inactivates protein synthesis, causing destruction of epithelium and necrosis. This leads to a pseudomembrane forming in the tonsilopharyngeal area, obstructing the airway and making breathing and swallowing difficult. The patient presents as severely ill.
