AFFIDAVIT OF LOSS

I, JOHN LESTER SALADAS, Filipino, of legal age, single, and a resident of

General Santos City, after being duly sworn to in accordance with law, hereby depose
and say:

1. That I am an employee of General Tuna Corporation, with office address

located at General Santos City;
2. That as such, my employer issued in my favor a company identification Card
bearing the no. 22070101;
3. That I had the said identification card placed inside my bag for safe keeping
and easy reference;
4. That lately, when I tried to retrieve the above mentioned card from where
it was safe-kept, the same was lost;
5. That despite diligent efforts to locate and retrieve the said card, the same
could no longer be found and is now beyond recovery;
6. That I execute this affidavit to attest to the truth of the foregoing for all
legal purposes.

AFFIANT FURTHER SAYETH NAUGHT.
