SUBJECT : DESIGNATION AS COVID-19 SCHOOL SAFETY OFFICER

In addition to your teaching loads/routinary assignment, you are hereby designated as Covid-19 School
Safety Officer of this school, effective immediately to perform the following duties and responsibilities, to wit:

1. Coordinate with the appropriate bodies for support and referral to community – based isolation
facilities for confirmed cases with mild symptoms, and to health facilities to severe and critical
care;

2. Undertake contact tracing or coordinate the conduct thereof;

3. Monitor status of students, stakeholders and employees quarantined or isolated; and

4. Implement return to work policies.
