AFFIDAVIT

I, M. SUMATHI, W/o. Late Mr. MARIMUTHU, aged about 41 years,

residing at No. 25/8, Gandhipuram 1st Street, Kannamapet, T. Nagar, Thiyagaraya
Nagar, Chennai-600 017, do hereby solemnly affirm and sincerely state as follows:

(1) I am the deponent herein.

(2) I state that I am an Indian citizen by birth.
(3) I am residing at the above said address.

(4) I state that I was born to my parents Mrs. N. VELANKANNI And Mr. M. NIRMAL
ANANTH on 25.06.1994 at Home and it was not registered by concerned authority due
to unawareness.
(5) I state that my original name is N. PRADHABAN which was mentioned in all my
revenue records.
(6) I state that my name was wrongly mentioned as MAHIMAI PRATHABAN in my
Register of Baptisms, Extract Form, Diocese of Vellore, Mazhuvankarunai.
(7) I state and declare that N. PRADHABAN and MAHIMAI PRATHABAN is identified
one and the same person.
(8) I state and declare that N. PRADHABAN is true and correct name of me.
(9) I swear this affidavit to be produced before concerned authority to whomsoever.
(10) I submit that all the facts stated above are true and correct to the best of my knowledge
and belief and nothing material has been concealed there from.
