Affidavit for permanent address acknowledgement

We, Mr. Sandeep Raj Kishor Singh and Mrs. Ranjana Sandeep Singh Maiden name
Ranjana Umakant Yadav, both aged 37 Yrs, Son of Raj Kishor Singh, & D/o Umakant
Yadav, Hindu by religion, professional & Government Employee respectively,
residing at C/O Raj Kishor Singh House No. 240, Post Bhawanipur, Takiya Bazaar
Samiti, Tehsil Sasaram, District Rohtas, Pin code 821 113, State of Bihar do hereby
solemnly declare and affirm as follows:-

1. That I myself & my wife having our Date of birth on 1 st March 1985 & 6th July
1985 respectively after my birth my permanent residence is at the above
mentioned address.

2. That our marriage was solemnized on the 30th November 2014 at New Delhi
and that our marriage was registered with the office of the Registrar of
Marriage. That out of the said wedlock one issue i.e. our daughter namely
 ____________________ is born. That from since solemnization of our marriage
I was joined by my wife as her matrimonial home. That my wife being in
Government service and the nature of her job is transferable we shifted at our
present residence in (complete address) Gurugram, Haryana.

3. That we frequently visit our above mentioned permanent address at regular

intervals as my parents continue to stay here.

4. That the above mentioned address of Bihar be henceforth acknowledged as

our permanent address of residence.

5. That I hereby furnish the Electricity Bills, (others) apart from this affidavit, we
have no other documentary proof in support of our residence.
