AFFIDAVIT OF UNDERTAKING

1. That I am presently enrolled at Ayala National High School

SENIOR HIGH as a Grade 12 student under the

2. that one of the requirements for the completion of Senior High

School is to take an 80-hour Immersion at

3. Having fully understood that all information on technology

process standards, quality assurance methodologies, production
capabilities, materials, purchasing, marketing, finance, and
all other related documents, manuals, operational and technical
matters that the _______________ shall make available to me
shall be used for the sole purpose of this Immersion. All these
data are classified as CONFIDENTIAL in nature. Thereby, I
hereby undertake to prevent the transfer of such information to
any party outside the __________________ . Failure to do so, I
should then be subjected to appropriate disciplinary action to
be imposed by the proper school authority;
