WORK SYSTEM DESCRIPTION
Description: Investigation of fire suppression system which got released in server room
System: Inergen System
Location: Server Room

Investigation Details:
Problem:
*What The Inergen cylinder got release during PPM in main server room

*When 4:58 AM

Investigation Analysis:

-Before we started the PPM of Fire Suppression system, I showed to my technicians all SOLENOIDS to be REMOVE inside Energy Centre aswell
as in the staff pantry nr Security which is the other inergen rack located.
- At First we started our PPM of fm200 and inergen Panels in BMS Corridor ( LV Rm1, Etisalat Rm, etisalat rm, DB rm, Control Rm and CBS rm)
And all these panel in this area are working properly without any issue.
- Then after finishing all the inergen cylinders in BMS Corridor, we proceed to Main Server Rm. During this time we started to test the Manual
release button in which the incident happened
* We investigate immidiately the inergen cylinder on why this cylinder activated.

Findings:
We found out that one solenoid is not remove in the cylinder and 11 cylinders released (from #11 to #22)

Cause:
- Negligence of technician, Since he is new from the site, he does'nt know yet the quantity & Location , etc. of this Inergen cylinders.

Damage Caused
1. 11nos inergen cylinders released
2. Ceiling Frames
Corrective Actions
1. Need to train the technician and familliarize the qty and the location of each systems
2. Need to refill the cylinders
