To whom it may concern,

This is to authorize _______________________ to the following act for and in my name

and sted to perform the following acts:

1. To represent me before for and in my behalf in processing the application for new
electric service with MERALCO, concerned Department of the Local Government
of Bacoor Cavite and any other concerned agencies/ institution of the Government;

2. To represent, sign, execute, pay the fees required, sign any and all document(s)
and paper(s) as well as claim or receive for and in my behalf said document(s)
from the above agencies, as well as to do any and all acts in order to claim the
aforestated application for new electric service;

3. To do all acts necessary to give effect the foregoing authority.

HEREBY GRANTING unto my said representative full power and authority to execute
or perform whatsoever requisite or proper to be done in about the premises as fully to
all intents and purposes as I might or could lawfully do if personally present, with
power of substitution & revocation and hereby ratifying and confirming all that my said
legal representative or his substitute shall lawfully do or cause to be done under and by
virtue of these presents.
