AFFIDAVIT OF LOSS

I, HIDAYAH ARUMPAC HADJI PALIL, of legal age, single, Filipino

and a resident of Balindong,Lanao del Sur, Philippines; having been duly sworn in
accordance with law, hereby depose and state:

1. That I am the holder of the said Notice of Admission for the LET
Examination on March 29, 2020 but due to PANDEMIC it was postpone
and reschedule for this June 26, 2022.
2. That I have recently discovered that the said NOA is irretrievably lost
and can no longer be found despite diligent efforts to locate the same;

3. That I am executing this Affidavit to attest to the truth of the foregoing

facts and to use the same for whatever legal purpose it may serve.
