Madam/Sir, : ‘You may be aware that the State Government shall be holding the 6* edition of Duare Sarkar with the organising of outreach camps and receipt of applications from 1* to 10" April, 2023 and delivery of services from 11" to 20 April, 2023 respectively. Under the 6" edition of Duare Sarkar, the Health and Family Welfare Department is providing services under the following schemes- Swasthya Sathi, Disability Certificates and Health check-up camps. 2.0 In view of the above, to ensure service delivery by 20th April, the application processing system must be in place from the first day itself. It must be ensured that all applications are processed on the same day as received, Adequate arrangements like appropriate scheduling of Disability Board screenings, etc must be made to meet the timelines for service delivery. It shall be seen that no applications lie unattended at any stage. 3.0 The SOPs for Swasthya Sathi, Chokher Alo and Disability Certificates are enclosed with this letter. It must be ensured that the SOPs are strictly adhered to with special emphasis on disposal of Swasthya Sathi applications. 4.0 In any cases of rejection of applications of Disability Certificates, the reasons for such rejection must be intimated to the applicants without fail. 5.0 Health check-up camps for screening facilities should be judiciously planned by the,CMOHs so asto cover all hamlets, remote and far-flung areas. It must be ensured that these camps and properly staffed and attended, 6.0 Vision screening by technical persons will be a component of the selected eamposite health ‘check-up camps under Chokher Alo. For this, the CMOH/HO-ULB should arrange vision screening for all age groups followed by service delivery, including spectacles distribution for the program eligible age groups and need-based referral services, g). rome 7.0 A VC shall be held on the above points with all ADMs( Health), CMOHs, MSVPs and Principals shortly Itis again re-iterated delivery of services is done within the prescribed timeline i.e. 20" April 2023 shall be in place. The vision screening sessions would be a component at the selected composite health screening camps at the forthcoming Duare Sarkar event. @the CMOH / HO-ULB would arrange vision screening followed by the service delivery, including the spectactes distribution and the need-based referral services, preferably at the chronically under-served (with the vision care services) areas with an aim for the maximum reach. 3. At these camps, the primary vision screening will be done for all the age groups by the CHO, ASHA and by other trained health workers, 4, The screened suspects with the vision impairment will be examined in detail by the Ophthalmic Assistants / Medical Technologists (Optometry) in the camps. 5. The need-based advised service circle, especially the distribution of the customized free-spectacles for the program-cligible free-spectacles age groups (School students of Class I to XII; Age 45 years and above), will need to be completed within following 10 days from the date of the advice given. The age-proof for the 45 yrs and above aged and for any easily available identity proof for the school children would be needed. 6. Each of the eligible spectacles-beneficiary will get a coupon, with official seal and signature, by the MT (Opto) or by the person authorized by the BMOH/HO-ULB. 7. The program eligible age group for the free spectacles would get the customized spectacles from a pre-announced date / time / venue, preferably from the same venue or from the nearby health facility/HSC/GP office, on production of the said coupon after 10 days from the advice for the same. The spectacles distribution would be on the pre-announced dates as follows :-—-advised on 1" April will get on 11" April, advised on 2™ April will get on 12" April, and advised on 10" April will get on 20" April positively. If the distribution day falls on a Sunday or any other Govt. holiday, the distribution would be on the next working day OR as per decision of local authority. 8, The deployment of the HR, especially the MT (OptoyOA, would be planned in such a way to keep the ongoing routine services least affected as far as practicable, ° . The suitable representative from the selected / tagged spectacles-vendors will remain present at each of the camp sites for taking the measurements/choice of frames ete. then and there. At the session end, the yendor will get the writ order with the line list of the detected program- eligible spectacles-beneficiaries, from the authorized Health camp-in- charge or from the MT (Opto)/Ophthalmic Assistant there. The vendor will nced to reach the customized spectacles, to the person/s authorized by the CMOH/BMOH/HO-ULB, in name-labeled beneficiary-wise boxes well in advance from the date of distribution so that the distribution can be done as per the schedule. 10.The fast-tracking of all the non-refractive error related services also, especially such as for the cataract surgeries etc., would be done as per the latest State guidelines. 11,IEC activities may be undertaken by local MoU NGOs by using suitable local level campaign methods/tools. Services of the ASHA,AWW,ANM, MAS members and the CHOs will need to be optimally utilized who would do this through whispering campaign, as they haye good acceptance at local levels. All the forms of the IEC / IPC activities would always clearly mention the eligible age group for having the need-based free spectacles to avoid any confusion and/or chaos for the same at the camps. 12.A team of at least two Ophthalmic Assistants / MT (Optometry) would do the vision screening per camp only for the primarily screened out suspects, The deployment of MTs/OAs will be preferred from same as well as from nearby blocks/health units. The MTs/OAs from the MoU NGOs would also be utilized to extend the honorary services. 13.This screening activity at these camps will be done by the MT (Opto) at the disposal of the respective CMOHs, The screening at the ULBs will be done by the OA / MT (Opto) at the disposal of the MoU-NGOs. This arrangements / allocation cic. will be finalized by the CMOH in consultation with the District Magistrate. 14,Arrangement of Necessary equipment/logistics for the vision screening/testing would be done by the MT (Opto)/OAs entrusted for the work. 15.The other basic drugs/logistics/eonsumables would be there as per the State guidelines for the Mission Mode Cataract Surgery Special Campaign activities. 16.The daily reporting will follow the general reporting system for the Duare Sarkar camps. Additionally, the ophthalmic services will be uploaded daily on to the CHOKHER ALO portal as the camp activity. 17.Concemed BMOH/Block authority and Pharmacist for rural areas and UPHC areas respectively will track/follow up each of the identified beneficiaries till the completion of the appropriate service-circle, 18.All the sessions will need to be kept under an intense supportive supervision and monitoring. CMOH will finalize the District level Nodal Officer for this campaign. Preferably the DPM-NPCBVI. ACMOHs will be the Nodal Officers for their respective sub-divisions. ACMOH, Sadar sub-division will assist the District level Nodal Officer on day-to-day basis in addition to his/her routine assignments. ns w ry = eo ~ SOP on issue of Disability Certificate through Duare Sarkar camps ‘The applicant will apply in a prescribed form which will be received at ‘Duare Sarkar Camp. The applications will be collected from BDO office by the concerned BMOH on the same day and send to the CMOH & Dy CMOH-IV ting it to CMOH & Dy CMOH-IV, a compiled report on nature of disability for which certificate is requested shall also be sent. electronically on the same day. CMOH will organize Disability Camps in the blocks or SDH or District Hospital or MCH within 5" day from the receiving applications accordingly based on compiled report received from BMOHs. . Concerned MSVP / Superintendent will arrange for disability board in consultation with CMOH and inform the date of board to the applicant over telephone and SMS both. Necessary arrangement to intimate the applicants shall also be made from BMOHs. . MSVP / Superintendent will upload data through Dy CMOHLIV in the Google sheet under Certificate delivered or rejected on the date of medical board. If an applicant is rejected, MSVP / Superintendent shall convey the reasons to him in form VIII within a period of 6 days from date of receipt of application. If Applicant needs any tests that will be done either on the same day or within a period of 3 days. Last date of issue of all certificates is 17" April 2023. Dy. CMOH-IV of the concerned district will act as Nodai Officer of Duare Sarkar Phase VI.
