As part of the learning competencies in the 4th quarter, the Grade 10 students are now
conducting research to relating socio-cultural issues under my tetulage and guidance. This
performance of the students aims to guide grade 10 students the nature of research and prepare
them for their senior high school and college research works. More so, in conducting this
performance they will be able to determine the socio-cultural issues in their enviroment so as
to look for possible solutions to it.

In connection to this, may I request your good office to permit the Grade 10 students to
conduct an in-depth interview with their respective participants. I assure that the participants
are informed on the nature and purpose of the study and whatever information/data will be
gained will be kept confidential.

INFORMED LETTER OF CONSENT TO PARTICIPATE IN THE STUDY

Dear Ma’am/Sir:

We are Grade 10 students at Pulungmasle National High School; taking our Research subject and
currently conducting our research title __________________________.

1. PURPOSE OF THE STUDY

This study aims to explore the reflection of the School Heads of Private schools on Value-based
Stewardship and seek recommendation on the program of Value-based Stewardship.

2. PROCEDURES

If you agree to participate in this study, you would be asked to do the following:
a. Listen to a brief explanation of the study and its purpose;
b. Sign an Informed Consent form;
c. Participate in an interview. The interview will take place at a time and venue that suits you so
as to minimize inconvenience for you as a participant.

3. PAYMENT FOR PARTICIPATION

Participants will not be paid for their participation in this study.

4. CONFIDENTIALITY

a. All information obtained in this study which can be identified with you will be treated
confidential and will be disclosed only with your permission.
b. Confidentiality will be maintained by means of coding procedures which will ensure that
all participants remain anonymous throughout the process. All data will be stored in a
secure location, with only the researcher having access to it.
c. Interviews will be videoed, and the participant has the right to review the video. Only the
researcher will have access to the video that will also be kept in a secure location.

5. PARTICIPATION AND WITHDRAWAL

You can choose whether to be a part of this study or not. If you participate in this study, you are
allowed to withdraw at any time without consequences of any kind. You may also refuse to
answer any question and still remain in the study. The researcher may advise you to withdraw
from this research if circumstances arise which warrant doing so.
