AFFIDAVIT

I, ELIZA F. SALIGUMBA, Filipino, of legal age, married, and a resident of San

Jose, Mahayag, Zamboanga del Sur, Philippines, after having been duly sworn to in
accordance with law, hereby depose and state, THAT:

1. I personally know HAZEL C. BRONGAN, who is the daughter of Rosita C.

Brongan and Reynaldo S. Brongan, for we have lived in the same Barangay
for a long time already and her family has been a close family friend;

2. I have personal knowledge that Hazel C. Brongan was born on January 14,
1993 at San Jose, Mahayag, Zamboanga del Sur, and was legally baptized by
Rev. Fr. Beda, sometime in the year 1993 at Senior San Jose Parish Church,
San Jose, Mahayag, Zamboanga del Sur;

3. I have personal knowledge of the foregoing for I was her godmother and I was
there on the day of her baptismal;

4. It was discovered only very recently that her baptism was not put into record,
such that her baptismal record could not be found at Senior San Jose Parish
Church;

5. In view thereof, it is my intention to request San Jose Parish Church, to put

into record the baptismal of HAZEL C. BRONGAN, and issue the baptismal
certificate;

6. I am executing this affidavit to prove the veracity of the facts mentioned

herein and for whatever legal intents and purposes it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 10th day of May
