AFFIDAVIT of CANCELLATION

I, JEAN B. LARA, of legal age, Filipino citizen, married and a resident of Zamora St.,
Molave, Zamboanga del Sur, Philippines, after having been duly sworn to in accordance with
law, hereby depose and say, THAT:

1. I am the registered owner of LARA TRUCKING SERVICES, with DTI Certificate

Number 03497512;

2. The said business establishment is originally located at Zamora Street, Barangay

Maloloy-on, Molave Zamboanga del Sur. However, as recorded in the Certificate of
Business Name Registration issued by the Department of Trade and Industry (DTI), the
place of the said establishment was erroneously written as Bogo Capalaran, Molave,
Zamboanga del Sur;

3. In view thereof, I hereby request the Department of Trade and Industry (DTI) to issue
another Certificate of Business Name Registration changing the address from Bogo
Capalaran, Molave, Zamboanga del Sur to Zamora Street, Barangay Maloloy-on,
Molave Zamboanga del Sur;

4. I am executing this affidavit to prove the veracity of the facts mentioned herein and for
whatever legal intent and purposes this affidavit may serve best.
