The supervisor assessment form is to assess Course Learning Outcome 3 and 4 (CLO3 and CLO4) of
the MEM564 course. The assessment is designed according to the MEM564 supervisor assessment
rubrics. This assessment is important to assess the students practical skills and teamwork.

Instructions:
This is a group work task. Students need to work in a maximum of FIVE (5) members in a group. Each
group must nominate their team leader. Each member of the group must work in a team in
completing the experiment.
