I, RONIEL O. ULAT ,Filipino of a legal age,single with residence of Mati City,

After having been sworn in accordance with the law,do i hereby despose and state that;

1. I am an employee of MECC SHOPPING CENTER CORPORATION..as

Officer in charge of the company, located at Limatoc st. Brgy.central Mati City

2. That i have filed voluntary resignation with the company affective date Dec 31,2021

3. That, I affirm and confirm that i was not forced into resignation and upon proper computatoin by the

Company’s Finance Departman, I am to recieve the amount of Thirty four thousand two hundred(34,200)
Phlippines Currency, as final and full settlement of all such separation pay,13th month pay,bunos and
employment concact provision;

4. Furthermore, I Voluntarily and freely acknowlodge that i have no claims or no right of action,and can’t
go back to my previous company in view of my voluntary resignation;

5.That this affidavit was executed for the purpose of attesting to the truth of the foregoing facts and for
whatever legal purpose it may serves best;
