I, “Neela Chettri”, aged about 30 years, daughter of Kharka Bahadur

Chettri, resident of Thingling - II, P.O. & P.S. Gyalshing, West Sikkim do
hereby solemnly affirm on oath and state as follows:-

1. That my actual and correct date of birth of is “20.01.1991” (Twentieth

January Nineteen Hundred Ninety One) which has been recorded in
my educational testimonials issued by Central Board of Secondary
Education.

2. That inadvertently my date of birth has been recorded as “15.01.1990”

(Fifteenth January Nineteen Hundred Ninety) in my Birth Certificate
bearing Registration No 233/02, Date of Registration 24.07.2002 issued
by Registrar of Births & Deaths, Primary Health Centre, Yuksum, West
Sikkim.

3. That only my year of birth has been recorded as 1991 in my Aadhar Card
bearing Aadhar Number 8155 2069 0870 issued by Unique Identification
Authority of India.

4. That this affidavit is sworn in to declare that my actual and correct date
of birth of is “20.01.1991” (Twentieth January Nineteen Hundred
Ninety One).

5. That this affidavit is made bona fide.
