INTRODUCTION

Br. C.A.T. International Co. Ltd. place paramount importance on Health, Safety and Environmental
issues. We continually strive to eliminate risk of accidents and incident and loss with regard to both
Client and C.A.T. Personnel and Property.

We are very proud of our long construction industry experience which spans some 82 years and our
very good record where Health, Safety and Environmental performance is concerned.

In order to continue to improve performance we embarked on totally modifying and updating our Health,
Safety and Environmental structure and were certified compliant with OHSAS 18001 by Bureau Veritas
on 20th December 2005. We have developed and implement an OHSAS Management System of
modular format that addresses all aspects of H.S.E. currently anticipated in our general scope of work
and also facilitates easy expansion should the need arise.

The Structure of The Management System is a proactive one with Planning and Preventive Action
being core elements through the initiation of regular Health and Safety Training & Instruction, Hazard
and Risk Identification, Health and Safety Audits and comprehensive analysis of Near Miss Incidents.

However no system is perfect and we also include Corrective Action combined with Root Cause
Analysis, Remedial Action, Lessons Learned and a comprehensive Reporting, Investigation and Data
Analysis system that should contribute to a dynamic system that is continually improving.

There are within the system various levels of documentation which are approved at differing levels
within our organization; the separate Structure Chart shows in detail these levels.

In addition to all C.A.T’s Safety Policies, Rules and Regulations all the applicable requirements as per
Saudi Aramco Construction Safety Manual (SACSM), General Instructions (GI’s) and other applicable
standards shall be complied with while executing this project.

This Project HIP shall be revised and amended as needed, after a major incident or occurrence, if
conditions change, new hazards are arise and if there is any change in scope of work. Moreover, the
thorough review will be used to monitor compliance with the program and the rate of success in
achieving its objectives and goals.

Work Method Statement: Methodology of execution of work with specific description and safety
protocols which identifies step by step procedure of work in correlation with each activity. The
statement generally includes the process for carrying out the task with the required resources and
prescribed responsibilities of involved personnel.

Hazard: A source or substance with potential to cause harm to people and property.

Risk: A risk is the likelihood that a hazard will cause a specified harm to personnel, property or
environment

Risk assessment: An identification of what within our work or workplace, which may have the
potential to cause harm to people or workers, so that you can consider whether you have provided
sufficient precautions or need to do more to prevent harm

Severity (Consequence): This includes the nature of the harm that may result from the hazard

Likelihood: is typically used to refer to events that have a reasonable probability of occurring, but
are not definite or may be influenced by factors not yet observed or measured.

Probability: The likelihood of a specific outcome, measured by the ratio of specific outcome to the
total number of possible outcomes.

This Hazard Identification Plan is formulated in conjunction with Saudi Aramco Construction Safety
Manual 5th Edition. It is Br C.A.T. International Co. Ltd. desire to anticipate and prevent all potential
hazards by identifying and providing preventive actions before the work execution. This plan
constitutes the written commitment of Saudi Aramco Project Management Team (SAPMT) and Br
C.A.T. International Co. Ltd., to administer or manage the construction of Berri Water Injection
System Expansion in a safe manner, with minimum risks to Saudi Aramco employees and facilities
as well as to the contractor’s manpower and equipment. Br C.A.T. International Co. Ltd. will ensure
that all work shall be carried out in accordance with Saudi Aramco Construction Safety Manual,
Schedule “D” and all relevant G.I’s and Saudi Aramco Engineering Standards (SAES).
Before starting work on the project site, especially inside live plant or facility, all existing U/G power
cables, communication cables, pipelines etc. must be clearly and physically highlighted on ground
Level with proper identification.

Scope of Work identifies the work required to support the Berri 250 MBCD AL Increment by installing
a remote water injection plant (SBWIP) at the southern part of Berri field with a total system capacity of
301 MBCD for reservoir pressure maintenance. The proposed facilities will provide water injection for
the southern and central part of Berri field.
The facilities shall be constructed I installed in accordance with this Scope of Work, standards &
drawings referenced herein.

The Work Includes;

Corrosion Inhibitor Skid


• Corrosion inhibitor Pumps with continuous dosing rate of 25 ppm.
• Corrosion inhibitor tanks with storage capacity of 30 days at maximum chemical dosing
pump capacity.
Biocide Injection Skid
• Biocide Injection Pumps with batch dosing rate of 1000 ppm.
• Biocide storage tanks with storage capacity of 30 days at maximum chemical dosing
pump capacity.
Firewater System
A fire water system will be provided for the water injection plant. The SBWIP is considered as a
non-process industrial facility. This system will provide firewater to fire protection devices such as
hydrants, and hose reels. The storage capacity will be suitable for four hours of flow and provided
by two dedicated storage tanks. The capacity will be provided by two firewater pumps without a
standby. One pump will be diesel-engine driven while the other will be motor-driven. Pressure in
the firewater system will be continuously maintained by two jockey pumps (1-duty + 1-standby).

Control Systems and Communication facilities


Process Instrumentation and control systems will be in accordance with the Saudi Aramco
Engineering Standard SAES-J-003. The project will install new Operator console for monitoring
of the new Water Injection system. This project shall provide data and voice network to the
Support Building and Gatehouse. This project shall also provide RFDT at the Gate House.

Potable Water System


The potable water system serves buildings and safety showers/eyewashes within SBWIP. It will
also provide utility water thus avoiding the need for separate utility water pumps. The potable
water system includes one storage tank, two (1-operating + 1-standby) pumps, and distribution
piping.
Raw Water Supply System
Firewater and potable water supply will be from two local deep wells. Each well will be equipped
with a submersible pump. A dosing system will inject chlorine at the following locations:
Common discharge header of deep well pumps
• Firewater system recycle line
• Potable water system recycle line
Drainage System
An oily water drainage system will be provided. The system includes catch basins/funnels, gravity
sewer piping, oily water sump, pumps and evaporation pond. The collected oily water will be
discharged to the evaporation pond, which is provided with oil skimmer in case of 8 mg/L of oil
content.
Two (1-duty+1-standby) normal flow pumps will be sized to accommodate drain flows during
normal operation or maintenance. Two (1-duty+1-standby) maximum flow pumps will be sized to
accommodate the greater of 1) flowrate caused by cumulative releases of drains during normal
operation or maintenance, plus the maximum firewater flow or 2) cumulative storm water flow,
based on a 5-year return frequency per SAES-A-112.

Wastewater Disposal
Septic tanks with either seepage pits or wastewater disposal fields will be used for wastewater
disposal.

• Project Manager:

• Project Manager shall report to the Area Manager regarding project related matters.
• Enforce C.A.T. & Saudi Aramco Health and Safety Policies and Procedures.
• Ensure proper coordination with SAPO.
• To ensure that a high level of H.S.E. Commitment is communicated to all employees.
• To demonstrate commitment by the provision of adequate resources essential to
comply with all project and legal requirements.
• Understand the Project requirements and Local laws with regard to H.S.E. and ensure
they are complied with.
• Ensure that through all phases of work are properly planned and meeting safety
requirements.
• Ensure all Supervisory personnel are qualified and receive required trainings.
• Provide all necessary resources to deal with any emergency situation or incident
occurrence.

• Project Safety Manager:


• The Project Safety Manager shall report to Area HSE Manager and will keep close
coordination with Project Manager regarding project HSE matters/concerns.
• Ensure implementation of CAT and SA HSE policies at project site.
• Ensure that all CAT employees and subcontractor personnel attend site safety
orientations, including other trainings as required by the SAPO.
• Ensure that all approved safety procedures are available and being implemented on
project site.
• Proper coordination with SAPO regarding HSE matters/concerns.
• Ensure in time reporting & investigation of all incidents.
• Assignment of safety personnel according to project requirements.

• Construction Managers:

• Construction Managers shall report to the Project Manager regarding project related
matters.
• Enforce C.A.T. & Saudi Aramco Health and Safety Policies and Procedures.
• To ensure that a high level of H.S.E. Commitment is communicated to all employees.
• Ensure provision of adequate resources to construction team for safe execution of work.

• Project Safety Coordinator/Supervisor:

• HSE Coordinator shall be reported to Safety Manager and keep close coordination with
Construction managers regarding HSE matters.
• Ensure implementation of CAT and SA HSE policies at project site.
• Assist Project Safety Manager in the formulation and review of documents and
procedures inclusive of Hazard Identification Plans, Emergency Response Plans, Work
Instructions and other documents that may be required to ensure a Safe and Healthy
Work Environment.
• Coordination with site safety staff and Area HSE Office.

• Project and Department Engineers:

• Understand the project requirements and Local laws with regard to H.S.E. and ensure
they are complied with.
• Ensure safest possible methodology is employed.
• Confirming that Work Permit Procedure is followed.
• Ensure that all company resources are being utilized in safe manners.
• Promote H.S.E. awareness at all levels.
• Set a personal example by working safely.
• Ensure all accidents and incidents are promptly and correctly reported.

• Construction and Support Group Supervisory Staff:

• Organize the site in order that activities are conducted on a safe manner.
• Communicate Hazard and Risk Information.
• Be aware of and communicate project and location specific H.S.E. requirements.
• Cooperate and assist H.S.E. personnel in their duties.
• Set a personal example to build a positive safety culture.
• Be familiar with and implement the Work Permit System.
• SA certified WPRs (work permit receivers) shall ensure safe execution of activities.
• Coordinate with other contractors / subcontractors on safety issues.
• Make sure that appropriate PPEs are available and used correctly.
• Ensure good housekeeping.
• Regularly check condition of tools and equipment.
• Conduct regular safety meetings.

• Safety Officers:

• Reported to site safety supervisor and close coordination with construction supervision.
• Understand the project requirements and local laws with regard to H.S.E. and ensure
they are complied with.
• Maintain accurate records of activities related to H.S.E.

• Liaise with SAPO representatives.


• Ensure only qualified & certified personnel are involved in different operations such as
Equipment Operation, Abrasive Blasting, Electrical Installations and First Aider etc.
• Monitor material handling and storage methods and ensure they are adequate.
• Verify that all activities relating to safety are carried out in accordance with approved
H.S.E. Plans, Procedures, Specifications, Laws, Regulations and Codes.
• Submit daily reports of activities to Project Health and Safety Supervisor.
• Promptly report and document all instances of nonconformance, incidents, unsafe acts
and conditions.
• Ensure that subcontractors and visitors are aware of and comply with H.S.E.
requirements.
• Ensure that all equipments are in safe condition & fit for use.

• Workers:

• Use appropriate PPEs and properly store after use.


• Report any situation that may be unsafe immediately to your supervisor
• The workplace is not a playground do not act in any manner that may endanger others
or yourself.
• Be aware of all safety rules and obey them.
• Obey all instructions and warning signs.
• Do not interfere with or operate any equipment without authorization which may
endanger others or yourself.
• Refrain from horseplay and abuse of equipment.
• Report all incidents to your immediate supervisor.
• If in doubt ask your supervisor or safety representative.

Hazard identification

Hazard identification is the process of identifying hazards in the workplace or for a work procedure. In
order to understand what hazard identification involves, it is first necessary to understand the nature
of hazards.

Following are the common types of hazards which we may interface at our workplace.

1. Physical hazards
A physical hazard is a naturally occurring process that has the potential to create loss or
damage such as;

• Noise
• Electricity
• Heat and cold
• Fire
• Floods

2. Mechanical Hazards
Mechanical hazards are created as a result of either powered or manual (human) use of tools,
equipment or machinery and plant such as;

• Crushing
• Shearing
• Cutting or severing
• Entanglement
• Drawing-in or trapping
• Impact
• Stabbing or puncture
• Friction or abrasion

3. Chemical hazards
A chemical can be considered a hazard if by virtue of its intrinsic properties it can cause harm
or danger to humans, property, or the environment. Health hazards associated with chemicals
are dependent on the dose or amount of the chemical such as;

• Toxic gases
• Noxious fumes
• Corrosive liquids
• Acid and alkalis
• Organic compounds
• Asbestos

4. Ergonomic hazards
Ergonomic hazards are physical conditions that may pose risk of injury to the musculoskeletal,
such as the muscles or ligaments the lower back, tendons or nerves of the hands/wrists, or
bones surrounding the knees. Ergonomics hazards include things such as;

• Awkward or extreme postures


• Whole-body or hand/arm vibration
• Poorly designed tools, equipment, or workstations
• Poor sitting arrangements in offices
• Repetitive motions
• Poor lighting

5. Radiation hazards
The emission of energy as electromagnetic waves or as moving subatomic particles, especially
high-energy particles which cause ionization.

Exposure to very high levels of radiation such as;

• Radiations from x-ray machines


• High powered lasers
• Radioactive materials and other electronic devices

6. Psychological hazards
Psychological hazards are hazards that affect the psychological well-being of people, including
their ability to participate in a work environment among other people such as;

• Physical stress
• Mental stress
• Fatigue
• Peer group pressure

7. Biological hazards
Biological hazards are also known as biohazards usually these originate in biological
processes of living organisms and refer to agents that pose a threat to the health of living
organisms, the security of property or the environment such as;

• Syringes containing potentially infected blood


• Specimen containers carrying potentially infected materials
• Legionella bacteria
• Viruses from air conditioning systems

Hazard Identification Methods:

There are number of methodologies which can be used in hazard identification process. Following
are few useful methods or techniques which can be used in hazard identification process.
• Walkthroughs such as to carry out pre-work site visit to know about the area and
proposed task.
• Analyzing the available data such as manufacturer data, incident data, scope of work,
work method statements and previously developed JSAs or risk assessments etc.
• Work site inspections carry out work site inspections and use their data in hazard
identification process such as management walkthroughs, audits, health and safety surveys
etc.
• Use of checklists is one of the useful tool for identification of hazards at workplace such
as daily, weekly and monthly inspection checklists.

1. Identify the Hazards:

Hazard or risk identification is part of the process used to evaluate if any particular workplace
condition, equipment, tools etc. may have the potential to cause harm to personnel, property
or environment.
Following are the key steps to identify the hazards but not limited to;

• Gather & review the information about the hazards present or likely to be present in the
workplace.
• Carry out initial and routine inspections of the workplace to identify new or repetitive
hazards.
• Collect the data from incidents investigations & illness records etc.
• Consider hazards associated with emergency or non-routine situations.
• Collect information from manufacturer data for the equipments/tools to be used at
workplace.

2. Evaluate and Assess the Risk:

Risk evaluation is to determine the consequences of hazards and then to decide the severity
level. Thus, the risk evaluation process is the combination of consequences and likelihood
of the hazards (Ref. to Below Risk Assessment Worksheet)

3. Decide Control:

Deciding control is a critical step in assessing the risk. After identification the hazards and
severity level of risk, it is required to decide suitable and reasonably practicable control
measures to protect the people, property and environment from harm.
While deciding control measures adopt ALARP (As Low as Reasonably Practicable)
technique.
• Elimination.
• Substitution.
• Engineering Controls.
• Signage/Warnings and /or Administrative Controls.
• Personal Protective Equipment.

Records shall be maintained showing the process of control assessment

4. Implementation of The Control:

Decided control measures to be implemented as per risk severity, such as some control
measures would need immediate attention, some may need additional resources or even
some hazards may need to be eliminated as reasonably practicable.

5. Review & Record:

HIP must be reviewed and findings should be recorded. However, beside periodic review
HIP shall be revised and amended, as needed, when conditions change, process change,
new hazards are introduced or the scope of work changes.

Evaluation Process

After deciding suitable control measures there should be continues monitoring or evaluation
process which will be helpful for periodic or necessary review of HIP.

Monitoring or evaluation can be done in the following way;

➢ Consultation with the employees & performing authorities


➢ Conducting periodic and routine inspections
➢ Management walkthroughs
➢ Coordination meetings with site supervision

Reasons for Review Reviewing

➢ When the results of monitoring (accidents, ill-health effects, environmental) are adversely not
as expected
➢ When interface with the new hazards
➢ When there is change in a method statement or process
➢ Change in management
➢ Change in legislation
➢ The introduction of new plant or technology
➢ As the time passes, risk assessment should be periodically reviewed and updated

Records

All records relevant to above procedure related to projects shall be kept by the Safety Department
in the site office; on the achievement of the Project, the Health and Safety records shall be
transferred to be archived and kept for a period of Five Years.
Records not related to projects shall be maintained by the responsible person and copied to the
Management Representative.

Following records shall be maintained but not limited to;

➢ Management walkthroughs
➢ Audit findings
➢ Incident review recommendations
➢ Accidents, ill-health effects & environmental records
➢ Job safety analysis
➢ Daily, weekly and monthly routine inspections
