New suppliers are required to complete this Supplier Questionnaire in order that your organisation can be risk
assessed and added to the Now Plastics Approved Supplier List.

Current Now Plastics suppliers are required to complete this Supplier Questionnaire every 3 years.

All suppliers need to complete Section 1 and sign the Declaration in Section 3.

Non-GFSI certified suppliers also need to complete Section 2. (For a list of Global Food Safety Initiative (GFSI)
recognised standards visit https://mygfsi.com/how-to-implement/recognition/certification-programme-owners)

All suppliers please attach the e-copies of the following documents:

A current specification for all products supplied or services supplied in the last 2 years

2.2 Product Safety Management System

to product safety, legality and quality, including layout, segregation,
building fabric and equipment standards, maintenance, chemical and
physical contamination controls, hygiene and housekeeping, waste

Your plan to ensure that vehicles used to transport products are kept
clean and maintained in good repair and condition, to protect products
from contamination.

Section 3 – ALL SUPPLIERS
3.1 Declaration & Authorised Signature
The Supplier:
a. Acknowledges that Now Plastics and its' customers will rely on the accuracy of the information provided for
product safety risk assessment of the supplier, the products and / or services it supplies.

b. Agrees that it will notify Now Plastics immediately:

i) of any significant changes to the supplier’s hazard and risk management plans before the changes are
implemented;
ii) if any external third party accreditations, regulatory approvals and / or registrations are suspended,
revoked or otherwise compromised;
iii) it withdraws or recalls any products manufactured at the manufacturing site (whether or not supplied to
Now Plastics), or any of its' customers withdraw or recalls any of their finished products as a result
(suspected, proven or otherwise) of any products manufactured at the manufacturing site;
iv) any criminal or civil legal action is started against it for product safety or quality matters;
v) any other significant changes that may affect the safety, authenticity, legality or quality of a product or
service supplied to or by Now Plastics.

c. Agrees that it will notify Now Plastics immediately if it becomes aware of any changes, errors or omissions in
the information provided.

d. Agrees that it will notify Now Plastics immediately if there are concerns about actual or suspected threats to
the safety, authenticity, legality or quality of a product or service supplied to or by Now Plastics due to any
incident or emergency that occurs while Now Plastics product is in its’ control that may be directly related to
the product, such as contamination, damage, or loss of load during transport, or indirectly related, such as the
disruption of key services including power and water, or environmental influences such as fire or flood.

e. Agrees that it will notify Now Plastics immediately of any non-conforming product identified whilst the
product is in the control of the supplier, whether the non-conformance has been caused by the Supplier or a
third party, and will clearly identify non-conforming product identified on site or notified to the supplier or
service supplier by Now Plastics to prevent its release, using either stock management IT systems, physical
segregation and / or physical identification, securely store non-conforming product to prevent accidental
release, and release, despatch or destruct non-conforming product only when notified Now Plastics.

f. Agrees that the site of manufacture of products supplied will not change without advance notification to and
the agreement of Now Plastics.
