RELEASE AND QUIT CLAIM

KNOW ALL MEN BY THESE PRESENTS:

WE, __ and __, both Filipino, of legal age, married, and with residence address at __,
Makati, do by these presents acknowledge receipt of the sum of TWENTY THOUSAND
PESOS (P20,000.00) ONLY, Philippine currency from ACCUSED as full and final satisfaction
of our civil claim arising from injurious acts due to our minor daughter, __ under existing laws.

In order to settle the incident amicably and since __ has offered the aforementioned sum
as fair settlement thereof, we are waiving all our rights to file any civil and criminal charges in
court against __. Further, we are withdrawing our all pending complaints against the latter.

By virtue of the foregoing, we do not have any further claim or cause of whatever kind
and nature against __.

We do hereby state and confirm that our acceptance of the P20,000.00 and the execution
of this Release and Quitclaim is our free and voluntary acts and deed and with full understanding
of the contents and effects thereof and knowledge of our legal rights.

IN WITNESS WHEREOF, we hereunto set our hands this 26th day of August 2022 in
the City of Makati.
