AFFIDAVIT OF ACKNOWLEDGMENT OF PATERNITY AND CONSENT

TO USE MY SURNAME

I, ANTHONY GARCIA MARCELO, of legal age, married, Filipino and

presently residing at Purok 1, Poblacion Sur, Talavera, Nueva Ecija, after having
been sworn to in accordance with law hereby deposes and states:

1. That, I am the biological father of ADRIANE VILLASAN MARCELO

(Adriane for brevity) who was born on January 14, 2002,
ALEXANDER VILLASAN MARCELO (Alexander for brevity) was born
on February 4, 2006 and MARY ANTOINETTE VILLASAN MARCELO
(Mary Antoinette for brevity) who was born on April 18, 2007;

2. That, all of my children, Adriane, Alexander and Mary Antoinette were

born illegitimate as a result of my relationship to their mother, MARY
ANN VILLASAN;

3. That, I hereby acknowledge my paternity to my children, Adriane,

Alexander and Mary Antoinette as my true, natural and legal heir to
Mary Ann Villasan;

4. That, I also give my consent for my children to use my surname

“MARCELO”;

5. That I executed this affidavit to affirm the truthfulness of the above

statements for whatever legal intent and purpose this may serve;
