I am currently working As a Company Site Representative for the witness of
all Well Services / Well Interventions Operation. Starting from the well
preparation after the completion of drilling phase. The work scope are
slickline operations, E-line operation for cement evaluation and perforation,
fracturing execution, coiled tubing, SMS operation and well testing.
My present duties as Well site Supervisor involve the co-ordination and
supervision of all operation as per below;
- Supervision of Snubbing unit operations including workover and completion
on HP gas wells
- Supervision of Multiphase unit operations on HP gas wells
- Supervision of E-Line set CBP and Perforation job.
- Supervision of PVT sampling jobs and transfer of samples
- Supervision of Fishing job, BHG and MPLT jobs with slickline on HP gas
wells
- Supervision of Coil tubing operation during well Frac/test unit outfit and
milling job, cementing job, kick off well job Sand clean out.
- Supervision of Wellhead maintenance and prepare handover report to
production.

contractors.

implement the annual HSE plans.
Promote and implement safe working practices for all operations and

against the AFE.

As a VESTFONN Stimulation Vessel Supervisor (Bakerhughes Co
Vessel) of the Multi purpose Vessel (Stimulation & Coil Tubing operation) at
ONGC, Mumbai Field in India. To perform Cementing, Gravel Pack,Acidizing,
Water Shut Off, Acid Frac and Propant Frac on Rig or through Coil Tubing..
My present duties as Vessel Supervisor involve the co-ordination and
supervision of all vessel crew and equipment during the planning, loading of
 the vessel, sailing to and from location and during the execution of
treatments. I also have to liase with the Customer Representative‟s
regarding the pumping schedule, submit chemical requirements, Vessel load
out requirements for the operation and ensure that the crew is fully aware of
requirements prior to starting treatments along with advising the Customer‟s
of equipment abilities and limitations. Additionally, I have to prepare and
submit daily operation reports to the customers office, and also BHI / BJ
office along with malfunction and non-conformance reports.

Other responsibilities are to ensure that all vessel equipment is maintained

and ready for use and to make sure that personnel are adequately trained
and competent to operate and maintain the equipment and that all BHI / BJ
and Customers safety polices and standard practices are followed and
adhered too. I also have to prepare annual appraisals for the vessel crew.

Acidizing job for both Unocal Indonesia and TotalFinaElf. Manage and
organize well all logistic (lab. Test, Chemicals, Foods, spare part etc) for
stimulation barge including crews for offshore work. Play important
roles to prepare all equipments, sparepart, training and barge operation to
deliver excellent Service Quality and QHSE of work and crews for Client.
Treating StimPAC Job using frac fluid YF140LG. The equipment are set up
on board OFS-01 with total 6000 HHP and treating Pressure 12,000 Psi,
using SBS613 POD Blender. The job were perform using Tree Saver
As a Frac Supervisor for Balikpapan district had performed all the job in
excellent Service Quality and make sure the client in happy after every
job by set up a post job meeting to review entire job sequences.

Indonesia and TFE with frac fluid YF100, YF100LG, HEC40 and
YFGOIII system. Pumping proppant from sand, Carbolite and Bauxite.
Also execute all Acidizing job for sandstone formation with Mud Acid and
Clay Acid System. Manage stimulation barge crew schedule, barge
logistic, spare part inventory, and all related barge operational issues.
Assigned as a CemCAT champion in Balikpapan.
Back to Unocal Oil Co.: In charge on Aquaria Barge (Stimulation Barge) as
a Treating Supervisor To perform Casing Clean and Frac and Pack Job and
as a Pumping Supervisor to Assist Coil Tubing Job to perform Gravel Pack
and Water ShutOff job w/ Maraseal on Kerindingan Field, Attaka Field and
Yakin Field. As Frac Supervisor for Balikpapan district mainly play roles
on running the stimulation barge, equipment preparation and handle the
peoples and logistic (lab. Test, Chemicals, Foods, spare part etc) in safe

Transferred to Off Shore Unocal Oil Co., in charge for every cementing and
pumping job on WorkOver (Snubbing Unit).
Unocal Oil Co.: As a Cementer in charge; Baruna I Rig (Barge Tender),
Baruna 3 (Work over Barge), Sarmex 2000 and Sedco 602 Deep water opr.
We installed the new Tech Unit on Baruna 1, Mecha Tronic Cement Unit and
ran the first offshore computerized cement Unit in Indonesia.
In charge of Galveston Key Rig (Santa Fe Drilling Company) for Unocal

Feb 1994 :
Joined Schlumberger (Dowell) as Service Technician 1 at the VICO
Indonesia. in Badak Field Operation.

During this time learning all the Dowell Shlumberger unit and technologies.

Job Description:

- Company Representative in charge and assigned of several locations, such as Unocal

locations (offshore, land), and TotalFinaElf Indonesie.
- Supervise our team for performing the job safely and successfully.
- Planning and preparing all materials and equipment for completing the entire job
requirement.
- Organizing equipment and materials for new locations or rigs, setting them up and
testing .
- Picking up as Tool Dresser Supervisor in town (Base) for cementing tools.
- Working on site as a team (drilling team, work-over team).
- Working with the team for bid tender documentations and technical designs for
cementing, that were then recommended to be a Schlumberger (Company) standard in
Indonesia.
- Spending time in cementing and stimulation laboratory and working as Lab technician.

ideas and suggestions
- Completing end of well reports for internal purposes and customers.
- Supporting the operation team from town; working with Cell Leader for a week every
hitch.
I am experienced for running Dowell down hole tool or packer, such as RBP, Positrieve
Packer.
2002 – 2003:
As a OFS-01 and LynLy Barge Well Service Loss Preventive Team (LPT) Vice-
Chairman. It is a part time job and selected by Management. LPT is a pro-active team that
assists in prevention and reduction of loss or damage to person, property and/or
environment. It has a shared responsibility with the line management for :
- Improve HSE awareness and safety, health conscious and environmentally responsible
working practices.
- Encourage active participation of the workforce in the risk identification program,
anomaly and safety talk TFE program.
- Ensure that new employees to operation have received a specific QHSE orientation.
- Recognize exceptional QHSE contributions from individual employees or teams.
- Prepare an annual site-specific QHSE plan.
