Currently, I am a full-time employee of this hospital, holding the position of Accountant I. I am

writing this letter in connection with my intention to engage in the practice of public accounting.
With such, one of the cited requirements by Professional Regulation Commission (PRC) in the
granting of accreditation for all government accountants, is to secure an “original copy of
authority to practice profession issued by employer and printed in the official letter head of the
institution/agency”.

National Convention and Seminar of the Government Association of
Certified Public Accountant, Inc. on May 23-26, 2018 at the Waterfront
Hotel, Lahug, Cebu City

Authority is hereby given to JAKE CHRISTIAN S.A. CHUA, CPA, Accountant I to

attend the 40th Annual National Convention and Seminar of the Government Association of
Certified Public Accountant, Inc. on May 23-26, 2018 at the Waterfront Hotel, Lahug, Cebu City
on official business.

Under this order, registration fee, transportation/perdiem and other incidental expenses
shall be charged against the appropriation of the Medical Center subject to the usual government
accounting and auditing rules and regulations.
