No. 2018 –

SUBJECT: Authority for Mr. Jake Christian S.A. Chua, Accountant I, to


exercise his profession / engage in the practice of public accountancy
outside office hours

In consonance with the existing Civil Service Rules and Regulations, Mr. Jake
Christian S.A. Chua, CPA is hereby authorized to exercise his profession / engage in the
practice of public accountancy outside office hours, subject to the following conditions:

1. He shall practice his profession outside office hours and during Saturday and Sundays
only;
2. He shall not use any government property directly or indirectly, nor make use of his
office in the performance of his outside activity; and
3. His practice shall be subject to the provision of CSC Rule IV, Section 22 (w) Discipline
and Rule X of Republic Act 6713, Sec. 1-C (Rule Implementing Official and Employees).

This authority to exercise this profession / engage in the practice of public accountancy
outside office hours shall be effective starting June 2018 onwards.

Moreover, this authority to practice his profession is subject to revocation and / or


cancellation the moment his practice is found prejudicial to the best interest of the government.

Annual National Convention and Seminar of the Government
Association of Certified Public Accountant, Inc. on May 23-26, 2018
at the Waterfront Hotel, Lahug, Cebu City

Authority is hereby given to JAKE CHRISTIAN S.A. CHUA, CPA, Accountant I to


attend the 40th Annual National Convention and Seminar of the Government Association of
Certified Public Accountant, Inc. on May 23-26, 2018 at the Waterfront Hotel, Lahug, Cebu
City on official business.

Under this order, registration fee, transportation/perdiem and other incidental expenses
shall be charged against the appropriation of the Medical Center subject to the usual
government accounting and auditing rules and regulations.

This Order is hereby on record.
