To Whom it May Concern; 
 
Nicholas Tschida, DOB 08/26/1988, has met the diagnostic requirements that qualify him for an 
emotional support animal. This animal is essential for him to function in his activities of daily 
living and he should be allowed to enjoy both his dwelling and travel accompanied by his ESA. 
 
He has been diagnosed with a condition that warrants an emotional support animal. He has 
undergone a diagnostic evaluation which has made this determination very clear. He is aware 
and has made many attempts to mitigate his condition with other means and the animal is the 
only measure of support that has been successful. 
 
Federal law protects individuals who meet criteria for mental health disabilities and Nicholas 
requires his dog for reasonable accommodation.  
 
I am fully licensed with the board of behavioral sciences and under federal law: 
 
Pursuant to The Fair Housing Act. In order to qualify for an assistance animal, the person 
making this request qualifies as having a disability which is: 
1. Having a physical or mental impairment which substantially limits one or more of such 
person's major life activities; 
2. Having a record of such impairment 
3. Such term does NOT include current illegal use of any controlled substance or rehabilitation 
from that thereof. 
 
ADDITIONALLY 
The assistance animal must meet the needs and assist the person in ameliorating the disability 
and promoting the healthy, vital function and performance of activities of daily living. The animal 
helps Nicholas to fully enjoy the premises by mitigating the symptoms of the disability. 
Activities of daily living involve those social, personal, work and recreationally related events 
which are vital for a person's mental and physical health, well being and overall functioning.  
 
 

 
AUTHORIZATION 
This authorization is made by a licensed mental health professional in the states of the 
person and or spouse noted herein, who can and is trained to diagnose, treat and evaluate 
clients who are suffering from behavioral health conditions. Therefore, as a fully licensed mental 
health professional, I am authorizing an emotional support animal for Nicholas. This 
authorization and verification is valid for one year from the date 7/3/2019.   
 
With this authorization, management of the premises must allow and afford the disabled person 
equal opportunity to use and enjoy the leased premises. Management is prohibited from 
charging pet rent or other fees generally associated and charged to persons with pets. 
Assistance animals are not pets but animals that are determined by competent licensed 
professionals to be an important and necessary treatment for or in assistance of a disability.
