PHYSIOTHERAPIST

TEMPORARY FULL TIME 12 MONTH CONTRACT


The Toronto Grace Health Centre (TGHC) provides specialized care and services to those individuals who require
Complex Continuing Care, Post-Acute Care Rehabilitation, Palliative Care, and Transitional Care. The care and
services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to
those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC
operates in 3 sites – the Main site (Church& Bloor St), Harbour Lights (Queen & Jarvis Street) houses and a
Specialized Care Centre (Finch& McNicoll)

Position Summary:
The Physiotherapist provides assessment and treatment for a mixed population. Within the vision, mission and
values of the Toronto Grace Health Centre, the physiotherapist assumes a leadership role in developing partnerships
with patients, families and health care professionals to ensure optimal patient-centered care delivery.

Responsibilities:
• In accordance with the standards of the College of Physiotherapists of Ontario and best practices: assess,
provide evidence-based treatment, utilize appropriate outcome measures, and facilitate discharge planning
for complex conditions and populations on the PACR unit, including but not limited to geriatric, transplant,
skin flap, orthopedic, and neurological populations; provide physiotherapy consultations on the CCC unit
and provide evidence-based interventions and patient/family education as appropriate.
• Evaluate the individualized care plan through discussion with the patient and/or family and in collaboration
with other members of the interdisciplinary team on an ongoing basis.
• Collaborate closely with the interprofessional team and communicate effectively with patients, families,
colleagues, and volunteers.
• Supervise Physiotherapist Assistants and assigns interventions based on the PTAs’ knowledge, judgment,
and skill level, while retaining overall accountability for care given.
• Supervise Physiotherapy students for clinical education.
• Act as a resource and advocate for patients, families, colleagues, and the Physiotherapy profession.
• Participate in the development and implementation of program and hospital initiatives.

Minimum Qualifications:
• Completion of a Bachelor’s of Science Degree in Physiotherapy or recognized equivalent.
• Registration with the College of Physiotherapists of Ontario in good standing.
• Current Basic Cardiopulmonary Life Saving course.
• Demonstrated excellent communication and interpersonal skills.
• Demonstrated critical thinking skills/problem-solving skills.
• Two years inpatient hospital experience in physiotherapy practice preferred.
• Previous experience working with adults with complex needs.
• Experience in the assessment and treatment of ABI CCC, critical care, cardio-respiratory and/or ICU, acute
neurological and orthopaedic, general medicine, experience an asset.
• Complies with the standards, codes and regulations of the College of Physiotherapists of Ontario and with
the legislation in the Regulated Health Professions Act.
• Excellent time management skills and demonstrated ability to perform in a fast-paced environment with
minimal supervision.
• Demonstrated computer skills knowledge of MS Word, Excel, Power Point and Outlook
• Excellent attendance record.
• Must have the physical strength, flexibility and endurance to perform the role as determined by the physical
needs of the position based on assessment conducted by Occupational Health Services.
