20 2.0 APPROVAL OF THE AGENDA OF THE MEETING

2 The presiding officer mentioned the proposed agenda and asked for any inclusion in
23 “Other Matters” prior to the approval of the agendum and there is none.

25 Ms. Candido moved for the approval of the agendum which is to tackle the Plans on
26 the rotation of the proposed process of the entire services of the office

30 The presiding officer asked the body on their suggestions/ recommendations about
31 their plans on the rotation of the proposed process of the entire services of the office.

33 Ms. Mabulac stated that a receiving clerk should be assigned in receiving all incoming
34 documents to ensure traceability of documents. She also added that a monthly meeting
35 should be done the end of the month and the presentation of the monthly
36 accomplishement.

38 Ms. Candido then added that not only the requested credentials should be included in
38 the monitoring but also the daily tasks of each staff in the office for the basis of the
40 accomplishment report. She added that the colleges/department with huge enrollees
41 should have two in charge to assure that the requests of the clients and students be
a2 accommodated. She further said that the one-day release must continue to ensure the
43 satisfaction of the customers.

Mrs. Pandong questioned the possibilty of the bulk requests of credentials like for
board examination of the education graduates and the enrollment. Ms. Mabulac suggested
strategizing the request to twenty or thirty requests per day to accommodate for education
when the time comes.

28 3.1 APPLICATION FOR GRADUATION
29 Ms. Candido asked the body on the process of the application and its schedule. Mrs.
30 Pandong answered that the process should be personal or by walk-in to assure that applicants’
31 needed or lacking documents must be submitted. The presiding officer said that the schedule
32. of the application for graduation is right after the midterm exam, the body agreed that the start
33 for filing is on March 28, 2022. Ms, Mabulac then said that posting of the schedule and
34 reminders with regards to the graduation must be done immediately.
35 3.2 ENROLLMENT TREND
36 The presiding officer asked the in-charge on the status of the enrollment trend. Ms.
37 Candido answered that itis stil on process since she just finished the report on the summary
38 of enrollment, She added then that the enrollment trend must be posted on the Facebook page
39 and it was agreed by the body.

41 Ms, Mabulac presented the number of clients and the number of requests catered by the
42. office for the month of February 2022. She said that the figure is still subject for enhancement
43 and it will be posted on Facebook page once itis finished.
44, 3.4 PLANS FOR ENROLLMENT
45 Ms. Mabulac inquired about the plans and process of enrolment. Ms. Abarratigue
46 suggested having face-to-face enrollment for the first-year students to ensure that the
47 documents to be submitted are exact and accurate. The presiding officer also stressed out
48 that the request for FORM 137 must be given to the students upon enrollment. However Ms.
49 Candido said that it is time-consuming since they also cater other services not only the
50 enrollment, she suggested creating a google form for the consolidation of first-year students
51 and their last attended school. It was agreed by the body.
52 Ms. Mabulac suggested to have the form for Student's Personal Information should be
53 uploaded to Facebook page for easy access of the incoming first year. She also asked the
54 approval of the body on posting the process for the enrollment and its requirements.
55 3.5 ROTATION
56 ‘The presiding officer asked the body on their comments and suggestions on the rotation
57 _ of the office. Mrs. Abarratigue inform that there should be duties and responsibilities for each
58 _ staff, she then added that filing of enrollment forms should be the priority for each college.
59 3.6 FORMS
60 Ms. Candido recommended to have the registered forms of the office be uploaded to
61 google drive so that other colleges can have access on the forms they need. It was agreed by
62 the body, and Ms. Pandong is the in-charge in uploading the forms to drive.
63 4.0 OTHER MATTERS

65 Mrs. Pandong asked action can be done for the late submission of the grade sheets. Ms.
66 Candido suggested to have monitoring of grade sheets on the UDC level aside from the in
67 charge, she said that this will also be used for the ISO and the like.
