Letter of Consent

Good day!

We invite you to take part in a research study. But before you decide, we would like you to
understand why and how you will be involved in this research project. We are therefore
providing you with the following information.

Please take time to read:

● We are from the College of Education of University of St. La Salle 1st year students and as a
requirement in our course, we are instructed to gather data for our research project “A Micro Case
Study” through conducting an interview with someone who has been in the teaching field for
more than 10 years.

● The duration of the interview will take an hour or more. You may choose if you are willing to be
recorded via video or audio only. Before any actions we are sure to ask your consent and
suggestions.

● A transcript will be produced; You will also be given a copy of the transcript and have the
opportunity to correct any factual errors.

● The record will only be limited to the researchers and the instructor. Overall this study is
Educational Purpose only.
