INFORMED CONSENT FORM

I am Jacel Mae J. Sausal, a Grade-12 Accountancy, Business and Management


(ABM) student of Puerto National High School, together with my groupmates Eden
Rose Silmaro, Charl Van Adora, Charise Palacio and Vienjell Villasor. As
candidates for the Senior High School graduation, we are conducting a research project
on “SENIOR HIGH SCHOOL STRANDS: FACTORS INFLUENCING THE STUDENT’S
PREFERENCE”. The objective of this study is to identify the factors affecting senior
high school students in their chosen strand. In view of the above rationale, we would
like to ask you to participate in this research study.

You are selected to be one of the respondents because you represent the
qualities that will best address the study. There is no risk to you in contributing in this
project. However, you are free to decide whether to take part or not in this survey. If you
decide to participate and choose to stop at your own free will, you can terminate your
participation. There will be no consequences, nor be punished for not participating.

The data that you will share in this study will be kept confidential. You will not be
named in any manner of publication and your identity is strictly private. Only the
researchers can access all the information you have given.

Confidentiality and Privacy of Data


All information gathered about you because of this study will be secured and
absolutely confidential. There is definite assurance from the researchers that
confidentiality, privacy and anonymity will be observed during and in the aftermath of
data collection, storage and publication of the research study. All data generated in the
process of the research study will be properly stored in paper or electronic form for a
period covering ten years after the research study has been completed.
