It has come to my knowledge that students of classes 6th to 12th are permitted to attend school for
regular classes from 6th Feb. 2021 with the written consent of parents.

to be allowed for attending regular classes scheduled from 8th Feb. 2021 in the school campus.

I solemnly agree for my son’s / ward’s strict adherence to follow the guidelines regarding hygiene and
sanitation issued by the school and also agree to send my son regularly to school in proper school uniform
on the scheduled days and time.

I also declare that my son is in good health and is not under any medication / medical treatment at
present. I also promise that in the incident of my son falling ill or having symptoms of Covid 19, I will
inform the school management immediately and stop my son from attending school with effect from the
same day.

I am also aware that in case of my ward / son getting any symptom of Covid 19 or having Covid 19
positive during the days of attending school, the school management will not be responsible for it.
