AFFIDAVIT

We, VILMA A. ELORO and ALVIE E. BIONG, both of legal age, Filipino
citizens, and residents of Brgy. Dalakit, Catarman, Northern Samar, after having been
duly sworn in accordance with law, hereby depose and state:

1. That we are the surviving heirs of the late ALEJANDRO G. ELORO, JR who
died on July 1, 2021 at Northern Samar Provincial Hospital, Catarman, Northern
Samar;

2. That the decedent left a bank deposit bearing account no. 1196258392 with an
outstanding balance of Php 254,712.31 in the Landbank of the Philippines,
Catarman branch, Catarman, Northern Samar;

3. That we are executing this affidavit to attest the truth and veracity of the
foregoing facts and circumstances and for whatever legal purpose this may serve
best.
