WAIVER OF CONSENT

(MINOR / PWD / PREGNANT WOMEN)

RULE
As a safety precaution, the following individuals shall sign a waiver before being allowed to enter the event premises:
– Persons ages 13-17 are highly encouraged to be accompanied by an adult ticket holder and they will be asked
to have their parent/guardian sign a waiver on show day.
– For ages 12 and below, they are required to be accompanied by an adult ticket holder (same section as them)
and will be asked to sign a waiver on show day.
– Persons standing four (4) feet below should present any government issued ID to prove that he or she is 18
years and above
– Pregnant women
– Persons with disability

As parent or legal guardian of _____________________, age _____, I hereby confirm that Minor/Pregnant
Woman/PWD (Special Ticket holder) has requested or voluntarily consented to enter the ___________________
(Venue), to participate in the ___________________________________ (Activity), organized by PULP Live World
Production Inc. (Promoter) or any of its related companies and “Co-Sponsor”, and that I have consented to the special
ticket holder’s participation in the Activities. I understand that the Activity involves certain risks of injury to the special
ticket holder, included but not limited to falling, colliding with others and with the barricades, tripping, losing balance,
and slipping on wet, slick and uneven walking surfaces. I attest that the special ticket holder is in good health and
physically capable of safely participating in the Activity.

In consideration of the consent given by the Promoter and Venue for the special ticket holder to participate in the
Activity, I agree on behalf of the special ticket holder to assume all risks associated with the Activity and hereby release
and hold harmless Promoter, Venue, Co-Sponsors and the related and/or affiliated companies and the officers, agents
and employees of each of them of and from any liability whatsoever for any and all claims, whether brought by or on
behalf of special ticket holder or myself, for any loss, liability or damage of any kind (including, without limitation,
personal injuries, death, loss of consortium, damage to or destruction of property, rights of publicity or privacy,
defamation or portrayal in a false light) and any related attorney’s fees and court costs (collectively “Damages”) arising
from or in connection with the Activity, including, without limitation, special ticket holder’s travel to and from attendance
at the Activity, and WHETHER DAMAGES ARE CAUSED BY THE NEGLIGENCE OF THE RELEASES OR
OTHERWISE.

I realize that (i.) this Liability Release refers to and covers events that may take place after the signing of this document,
and that the exact nature of any injury or loss special ticket holder may suffer as a result of his/her participation in the
Activities may not be entirely foreseeable, (ii.) the terms of this Release mean that I may be waiving certain rights, and
(iii.) if any portion of this Release shall not be affected or impaired in any way and shall continue in full legal force and
effect. I further warrant that (a) I HAVE VOLUNTARILY EXECUTED THIS RELEASE OF MY OWN FREE WILL,
WITHOUT DURESS OR PRESSURE FROM ANY PERSON, ON BEHALF OF THE SPECIAL TICKET HOLDER, (b) I
UNDERSTAND AND ACKNOWLEDGE THAT BY SIGNING THIS RELEASE I AM GIVING UP MY LEGAL RIGHTS,
INCLUDING THE RIGHT TO RECOVER DAMAGES IN CASE OF INJURY, DEATH OR PROPERTY DAMAGE; (c) I
HAVE READ THIS ENTIRE RELEASE CAREFULLY, AND I FULLY UNDERSTAND ALL OF ITS TERMS AND
CONDITIONS; AND (d) MY SIGNATURE BELOW IS AN ACKNOWLEDGEMENT THAT I HAVE HAD AN
OPPORTUNITY TO CAREFULLY READ THE ENTIRE RELEASE AND TO HAVE ANY QUESTIONS ANSWERED TO
MY SATISFACTION.
