PART 1: IFORMATION SHEET

Dear Parents/Guardian,

Good day!  We are conducting a study entitled "The Relationship of the Use of Mobile
Phones and Performance in English Class in Junior Hath School in Pugo Catholic School for
the School Year 2022-2023."  We the researchers:  Aleah Ashley C. Palcon, Jimrey M.
Fernandez, John Rain B. Namolngo, Kjerstine Belle G. Llobrera, Maria Eliza P. Santillan,
Nicole Anne M. Loredo, and Ranulf. Jay Estabillof Jay G. Estabillo request your child to
participate in this research. The Purpose  of the study is to determine the effect of mobile phone
use on students' performance in English class, as measured by their grade in the subject and
survey questionnaires.This study will be conducted to the whole Junior High students at Pugo
Catholic School, Pugo La Union. Google Forms, an online survey tool, will be used to carry out
the survey.

Procedures

Your child will receive a Google Forms survey questionnaire online, and the results of his/her
responses will be displayed. Despite the fact that he/she consented to participate, his/her
participation is fully voluntary, and they are free to withdraw at any time. In case they do not
wish to answer a question, they do not have to provide a reason; however, if doing so makes you
uncomfortable, they may do so. The study will not have any consequences if they choose not to
participate. No question has a correct or incorrect answer. Feel free to share their views and
opinions with us. Your child will not be exposed to any dangers as a result of the study.

Privacy and Confidentiality

We will protect your child identity, and all of your information will be treated with the
utmost respect and confidentiality. We will not make your confidential information public or
intentionally disclose it to any third party without first obtaining your authorization. In the
event that confidential information is lost or stolen, we agree to promptly notify you and take
responsibility.
