JOINT AFFIDAVIT OF ARREST

We, PO1 ANTONIO SINAG and PO1 ROBIN ROSANO, both members of the
Philippine National Police, presently assigned at the Cardona Rizal, Police Station, after
having been duly sworn to in accordance with law, do hereby depose and state that:

01. At around 10:00 in evening of 06 November 2022, JUAN HERRERA-

GONZALES 21 years old, Filipino, single, college student, residents of 234 San Roque,
Cardona Rizal, appeared at the Cardona Rizal Police Station to report a murder case
that transpired at around 9:30 pm in the evening at the harap ng Patunhay Calahan,
malapit sa elementary school. A copy of the Police Blotter Report dated 06 November
2022.

02. Said operation stemmed from the complaint of Juan Herrera-Gonzales

for accusation of murder which was referred to our office by the Cardona Rizal, Police
station against Sierra Disidiro-Makulong, (20), (single), (Filipino) and residing at Morong
Rizal;

03. On the complaint dated 06 November 2022, stabbing Eliot Cierro-

Lumalaban in the Street of Pat. Cal.

04. Upon receipt of their said complaint, our office planned out an
entrapment operation against the suspect. At about 7:00 of 07 November 2022.

05. Sierra Disidiro-Makulong who come willingly without speaking any

word, but smirking the whole time.

06. Seized and recovered in the possession and control of the arrested
person were the following, to wit:

a.) A knife that stab the victim (Evidence)

07. Suspect together with the seized and recovered evidence was brought
to the Cardona Rizal, Police Station, for booking and proper disposition. Likewise, the
arrested person was apprised of their rights as provided for under the Constitution of the
Philippines.

08. The report of the PNP Crime Laboratory Group Physical Identification
Report No. 12345 dated 14 November 2022, revealed that suspect Sierra Disidiro-
Makulong was POSITIVE for the her presence of the CCTV near Street of Pat. Cal.
 We executed this joint affidavit to attest the truthfulness of the foregoing facts
and to support the filing of Criminal Cases against Sierra Disidiro-Makulong for
Murder case.
