Canada Temporary Resident Visa

• Completed draft application forms

o IMM 5257 (TRV form)
o IMM 5707 (Family Information form)
o IMM 5475 (Authority to Release Personal Information)
o IMM 5476 (Use of Representative)
• Two (2) passport size photos (white background, no eyeglasses, w/ collared
shirt/blouse, both ears exposed, neutral expression)
• Current and old passport
• Photocopy of visas and stamps
• If employed, please submit Certificate of Employment and copy of recent ITR
• If self-employed, please submit copy of business documents such as SEC
registration, BIR registration, DTI, permits and latest Income Tax Returns
• If available, copy of proof of assets such as but not limited to; land title(s),
investments, bonds, etc.
• For students, please submit Certificate of enrolment and copy of ID
• PSA Birth Certificate
• PSA Marriage Certificate, if applicable

Additional Requirements
If you are going to visit family or friends, submit the following:
• Letter of invitation
• Copy of citizenship or immigration status document (to demonstrate that
they are legally staying in Canada) of the person inviting you
• For the person who will pay for your trip, please supply supporting financial
documents to prove that they can afford to support this visit, including
reliable and verifiable information from a third-party source of your host’s
income, employment letter and proof of funds

• Guarantee letter from the local company together the company documents
such as SEC registration, ITR and company Bank Certificate
