coach, I think it is important to create and conduct interviews to gauge exactly what your client
wants, and needs are. Questionnaires are a good way to do this and allow your client to disclose
any important and relevant information you may need to know. In a spiritual questionnaire,
some things that may come up are past experiences, triggers, desire for improvement, self-
evaluation, and explanation of their reason why they want to embark on their spiritual journey.
Once you understand who your client is and their desires, you can use it as a baseline to build a

considerations through thorough questions so you can ensure not to offend a client or make them
uncomfortable with your recommendations. There may be individuals who do not partake in any
religious or spiritual rituals, ones that are strictly religious to their own practice, and individuals
who are open to practice everything. These considerations prepare you for proper research and
opportunities to connect deeper with a client. Participation waivers must be included before
offering up any wellness plans to protect yourself and your client. The waiver is a way to state
your credentials and free you from any medical fault as you state what you are not qualified for.
This gives a client the opportunity to either voluntarily continue to the next stage or to decline
the service.

After completing the interview, I would include multiple choice questions to gather more
data on the client. It is important to have space for open ended questions that allow clients to
truly express themselves which give you a deeper understanding of them; it is also important to
have questions that allow a client to agree or disagree with to gauge common information to use
later on. As mentioned previously, religious considerations would play role into the
questionnaire.

Yes, I am now at the point in life where you value life. Caught the understanding early
on, using the answers to live life accordingly. Knowing brings happiness.
Yes, even when things are bad. When I came home today even though the house was a
