JOINT – AFFIDAVIT OF
TWO DISINTERESTED PERSONS
(for the purpose of the correction of entry in the Certificates of Live Birth of his children)

We, -O and -p, both of legal ages, Filipinos, and both residents of Daanbantayan, Cebu,
Philippines, respectively, after having been duly sworn to in accordance with law, do hereby depose
and say, THAT:

1. We personally know -c, who is the son of -j and -e because we have

been acquainted with him and his parents for quite some time already
and also neighbors in the same City.

2. In the entry of information in the Certificate of Live Birth of -c, the sex is
indicated as female instead of male;

3. That we know for a fact that he is male and not female;

4. That the inadvertent and erroneous indication of the mother’s first name
“-jj” is a mere clerical error which was not corrected immediately and the
correct spelling and designation of the said name is “-j”;

5. The father’s first name is written as “Esir--ee” and not “-e” as indicated
in his valid proof of identification such as Philhealth ID;;

6. We are not in any way related to -e, either by affinity or consanguinity.

7. That finally, we are executing this affidavit for the purpose of correcting
and changing his sex, mother’s first name, father’s first name in the
entry of information in his Certificate of Live Birth and for whatever
purpose it may serve.
