SEXUAL HEALTH

Butler University: Women’s Health Elective

Presented by: PGY1-Resident Morgan Dermody, PharmD, MBA

SEXUAL HEALTH

• Goals for today’s discussion:

• Understand sexual health in the context of women’s health
• Explain sexual health topics in the context of women’s health
• Apply discussion to a patient case on sexual health
WHAT IS SEXUAL HEALTH?

Poll Everywhere:

SEXUAL HEALTH

• According to the CDC, the World Health Organization defines

sexual health as:
• “[…] a state of physical, emotional, mental and social well-being in
relation to sexuality; it is not merely the absence of disease, dysfunction or
infirmity. Sexual health requires a positive and respectful approach to sexuality
and sexual relationships, as well as the possibility of having pleasurable and safe
sexual experiences, free of coercion, discrimination and violence.”

Centers for Disease Control. Sexual Health. 2019.

• Lisa is a 22 year old female who is coming to
your clinic today seeking a reliable form of
contraception.
• You, the pharmacist, inquire more about her
sexual health history, contraceptive
preferences, and other pertinent medical
S E X UAL HE A LTH: C A S E information.
A PPL IC ATION • Lisa explains that she is in a stable
monogamous relationship and is not always in
the mood to be intimate with her partner,
Apply your knowledge of sexual health however, “I do it anyways because he insists,
to the following case information: even when I’m not in the mood. I just don’t
want to get pregnant right now… Maybe
someday.”
• She goes on to explain she desires a highly
effective form of contraception, has not
received any recent vaccines she can
remember, has no allergies, and has never had a
pap smear.
SEXUAL HEALTH

• No, Lisa is not experiencing positive sexual health in her current

situation.
o Someone coercing their partner into sexual activity, even in committed
relationships, does not fit the definition of sexual health.
o Sexual experiences must be free from coercion in order to be considered
sexually healthy.

Centers for Disease Control. Sexual Health. 2019.

SUPPORTING SEXUAL HEALTH

• How do you address a situation like Lisa’s?

o Offer emotional support, utilize active listening to show her that her concerns are heard and allow her
the chance to open up about her experiences.
o Encourage the patient to seek out emotional help.
– Remember, emotional wellbeing and mental health are KEY aspects of sexual health,

HYPOACTIVE SEXUAL DESIRE DISORDER
(HSDD)

• DSM-IV-TR defines HSDD as “a • Updated DSM-5 criterial merged two

sexual activity, which causes marked disorder, thus eliminating HSDD and
personal distress or interpersonal female sexual arousal disorder.

Flibanserin:
• Brand Name: Addyi
• FDA Approval: 2015
• MOA: Agonism of serotonin 1A (5HT1A) receptor
and antagonism of serotonin 2A (5HT2A) to
reduce serotonin inhibition of excitatory
neurotransmitters. This mechanism produces
indirect increases in the release of dopamine and
norepinephrine.
• ADE: dizziness, somnolence, nausea, fatigue, Image 1
insomnia, dry mouth, hypotension, and somnolence
• Interactions: metabolized by CYP3A4 and 2C19

Bremelanotide:
• Brand Name: Vyleesi
• FDA Approval: 2019
• MOA: Agonism of melanocortin 4 receptor
(MC4R). Bremelanotide’s exact mechanism of
improving HSDD is unknown; however, it is
thought to enhance dopamine and norepinephrine
activity to increase excitation.
• ADE: nausea, flushing, headache, vomiting, injection
site reactions, transient increases in blood pressure

• Interactions: Significantly decreases absorption of
oral naltrexone, concurrent use not recommended

PREVENTATIVE MEASURES
SEXUAL HEALTH AND PREVENTION

• Another piece of sexual health is the prevention of disease.

o In women that prevention is often in the form of cervical
cancer prevention.
• The absence of disease does not define sexual health alone,
it remains a key component of sexual health.
• Preventative screenings for cervical cancer in women
include:
o Papanicolaou smears (Pap test)
o Primary HPV Testing

SEXUAL HEALTH: HPV

• Human papillomavirus (HPV):

o 200+ types of HPV exist
o Double-stranded deoxyribonucleic acid viruses (DNA viruses)
o Responsible for almost all cases of cervical cancer.
– HPV 16 accounts for approximately 50% of all HPV related cervical
cancers
– HPV 18 accounts for approximately 20% of all HPV related cervical
cancers
o Responsible for vulvar and vaginal cancers, genital warts, anal cancers,
oropharyngeal cancers, and even penile cancers.

• Prevention strategies include:

• Consistent use of barrier
methods (i.e. condoms, dental
dams) to prevent against
physical transmission of HPV
between partners.
• Administration of an
appropriate vaccine series to
prevent against HPV infection. Image 3

SEXUAL HEALTH CONCLUSION

• Sexual health is a large field of healthcare, encompassing the entire person

o This includes physical, emotional, mental and social well-being in relation to
sexuality.
• Pharmacists must ensure they are aware of multiple aspects of a patient’s care
opportunities, not just medications.
o Stay up-to-date on preventative screening recommendations for patients.
– Opportunities to counsel patients on these recommendations will continue to
grow as pharmacists see more patients for contraception and vaccine
administration.
o Be knowledgeable on vaccine series schedules and recommended dosing intervals.
o Be able to point patients in the appropriate direction for sexual health resources.
