1. Decide one school name where your CAR is going to be conducted, and describe
your reasons for choosing the school.
2. CAR will never be carried out without problems identification in teaching,
please identify the problems as the main condition for CAR.

3. From the problem identification, please formulate a title for your CAR
4. Please describe what theories will you use in your CAR and describe why you
use the theories.
5. Write your research background in at least 8 paragraphs which contains
information on the research problem.
6. Please formulate a hypothesis which describe the significant change which will
happen after your CAR is conducted.
7. What circles will you use to carry out your CAR and also describe what you will
do in each of the circle?
