MEMORANDUM OF AGREEMENT
(USE OF MOTOR VEHICLE)
KNOW ALL MEN BY THESE PRESENTS:
I. PARTIES
 This Memorandum of Agreement (Use of Motor Vehicle), executed this ___day October, 2017 in Quezon City,
Philippines, by and among:MR. VACLAV HABAN, of legal age, with postal address at _______________
hereinafter referred to as the “OWNER”;
 -and- _____________________, of legal age, with postal address at ___________________________ and ___
___________________, of legal age, witghpostal address at ____________________________ hereinafter
referred to as the
“HOLDERS”;
 
WITNESSETH THAT:
WHEREAS, the OWNER is the _______________ of _________________Corporation,
hereinafter referred to as “CORPORATION”
 registered under thelaws of ______________;WHEREAS, the HOLDERS are employees of above mentioned
corporation;WHEREAS, the vehicle subject to this Memorandum of Agreement (Use ofMotor Vehicle) is a
___________ (Model and Year), covered by OR/CR Number,
hereinafter referred to as “VEHICLE”;
 
II. UNDERTAKINGS
A. That the OWNER hereby allows the HOLDER to the use of the VEHICLE forany lawful purpose, during
reasonable hours and within reasonable locations,for the service and benefit of the CORPORATION;
B. That the HOLDERS shall inspect and provide the necessary informationregarding the condition of the
VEHICLE especially if there are defects noted, tothe management of the CORPORATION before he shall use
the same;
C. That the HOLDERS bind themselves to return the above-mentioned VEHICLEin the same condition prior to
its use or to pay the necessary dues to restore thesame to the said condition;
D. That the OWNER, under this instrument, hereby allows any other person asdetermined by the management
of the CORPORATION, aside from above-mentioned HOLDERS, to the use of the VEHICLE covered by this
Memorandumof Agreement;
