AFFIDAVIT OF CONFIRMATION OF SALE

             We, _____________________________, and __________________, both of

legal ages, Filipino Citizens, and residents of Brgy. ___________, ___________, Leyte
after having been duly sworn in accordance with law, hereby depose and say:

1. That we are the legal Heirs of the late ____________________,

respectively, who executed an unnotarized DEED OF ABSOLUTE SALE OF A
PORTION OF LAND with one __________________, vendees, on 21 st of January
1980 at Ponpon, Leyte;
2. That the subject property is situated at Brgy. ____________, Leyte, which
is a residential land consisting of 64 square meters, more or less, contained under Tax
Declaration No. ______________ declared under the name of ________________
and is particularly described as follows:

“A parcel of residential land designated as Cadastral Lot No. ___

situated at Brgy. ___________________, Leyte, declared in the name of
____________________, bounded on the North, by __________; South,
by _____________; East, by ______________; and West, by
________________, obtaining an area of 183.52 square meters, more or
less, with an assessed value of Php 2, 350.00, Philipine Currency.”

3. That we have personal knowledge of the fact that abovementioned

property was absolutely sold to ______________, by _________________________,
respectively, with the consideration of the total amount of FIVE THOUSAND PESOS
ONLY (Php 5.000.00) on 21 January 1980;

4. That the document evidencing Deed of Absolute Sale executed by

__________________________ was lost and could not be found but we attest to the
due execution thereof by the said parties;

5. That we hereby ABSOLUTELY WAIVE, CEDE and QUITCLAIM whatever

legal rights and interests we have had to the above-described property and all its
improvements thereon, in favor of ______________, their heirs and assigns;

6. That we are executing this affidavit to attest to the truth of the foregoing
facts and for all legal intents and purposes this declaration may thus serve.
