Here's what you need to know:

2. REQUIREMENTS:

2.A. Proof of Identification

(Please bring two photocopies of the valid ID - front and back)

COMELEC requires identification that shows both your name and proof of residence,
so please bing an ID with the following:

Valid ID * with current address

*The following IDs are examples of this:

Driver's License, NBI Clearance, UMID Card, Postal ID, PWD ID, Senior citizen ID,
Certification of NICP

**The following are also allowed: Philippine Passport, School ID, Company ID
but will require supporting documents as proof of residence: Lease contract, Proof of
billing, Registration form for College and Library card for HS

2.B. Download the registration form here or we can email it to you along with this
document

2.C. Download ____ (additional form - as needed - based on voter profile - eg assistor
form) here or we can email it to you along with this document

2.D. If you are required/allowed to bring an assistor, he or she must be either a relative of
sufficient age or a member of your household. The assistor must bring any Identification
card with name, photo, and address.
3. REMINDERS:

3.A. You may already fill-up the form using a pdf editor or manually fill-it-out using a
black or blue pen. However, DO NOT SIGN the Voter Registraion Form yet. The signature
shall be affixed and the oath shall be taken, upon personal appearance of the registrant
at the OEO, during the filing of application and biometrics data capturing

3.B. You must print the Registration Form three times and must be printed back and to
back using a legal sized bond paper???

3.A. Download the CORONAVIRUS SELF DECLARATION FORM here or we can email it to you
along with this document.

3.B. Contact your local Comelec office to check if you need to submit and ONLINE
APPOINTMENT FORM and ask about their schedule for Voter Registration.

3.C. Practice Social Distancing. Wear your face mask and face shield, and bring your
own bottle of water and hand sanitizer.
