for Acceptance of Different Specimen Signature

for Account Operation

I, _______________________________, holding CNIC No. _________________________ do hereby state

that:

1. I have shared a Signature Specimen (“SS”), which differs from my signature on my CNIC (hereinafter referred to as
my “Alternate Signature”).

2. It is requested to accept my Alternate Signature, as appearing on the SS, to be valid, binding and of legal affect since
I am desirous of operating my new account with United Bank Limited (my “Account”) with the Alternate Signature. I
undertake to be solely liable for any loss, damages, fines, penalties, claims, etc. incurred by the Bank or any related
party due to a difference in my signature as appearing on the SS and my CNIC.

3. In consideration of your accepting my Alternate Signature on all cheques, orders for payment and other written
instructions and / or requests as may from time to time be communicated by me with respect to transactions and / or
operations to be carried out in respect of my Account.

4. I hereby irrevocably undertake to indemnify and keep United Bank Limited, its officers, employees, directors and
assignees harmless and protected against any and all fines, penalties, losses, damages, detriments, claims, demands,
litigations, costs, charges and expenses whatsoever which United Bank Limited may, at any time, incur and / or sustain
as a consequence of placing reliance on my Alternate Signature.
