The class of NSTP CW1/LT1-(Section) from De La Salle University Manila under (Name of Social Engagement
Lecturer) is going to conduct a Community Service Activity for (name of partner community and address) on (date)
as part of the course requirement.

After a series of consultations with community stakeholders and our class, we have agreed to offer (type of
resources e.g. school supplies, medicines, art materials, hygiene kits, medical supplies, printer, grocery
items etc) to Name of Partner Organization and location/address of the partner community. However, as
much as we would like to help all of them, we cannot do this on our own. Thus, we need as much help as we can get.
