FOURTH QUARTERLY ADOPT-A-SCHOOL COORDINATOR’S
ACCOMPLISHMENT REPORT
S.Y 2021

I. Introduction

The Adopt a school program undergo a school process by means of the following: A Memorandum
of Agreement (MOA) will specify the details of adoption between the adopting entity and the head of the
school concerned. MOA is subject to review and approval of the School Superintendents. A standards
and guidelines for review of
adoptionformulated by the Coordinating Council (assessment for the application for tax shall be
credits by the adopting entity) The agreement shall last for at least two years. Schools that make it to the
list are the less privileged, those who have limited resources but have very large student population. Most
of them experience classroom shortage, insufficient textbooks and instructional materials and
overcrowding. Schools in low-income municipalities, those that are poor but have high performing
students and those with undernourished students are also given due considerations.
Oogong Elementary School they have supportive stakeholders that help them in the
achieving their goals for the learners. They are the partners in Brigada Eskwela and other
programs and projects that the school undertake.

II. Accomplishments

With this report, are the following accomplishments for October to December, School Year 2021

* Received donations from the stakeholders

* Made summary report of the donations received

* Uploaded reports in DPDS

III Problem Met

* There is no proper endorsement of the documents from the teachers received the donations
