THIS IS TO CERTIFY that based on the Police Blotter of this Police Station, as
shown on page , under entry No. 025, dated May 11, 2023, the following entries of
events exists, to wit:

May 11, 2023/1700H/C2023-025

SERIOUS PHYSICAL INJURY

Maria Bilaro, 21 years old, single, student and a resident of P-2 Kubo, Tabaco City came to the office and
reported the she was allegedly physically harmed by her ex-boyfriend Albert Colli, 29 years old, self-
employed and a resident of P-3 Tayhi, Tabaco City. Inquiry revealed the at about 3:20 in the afternoon of
May 10, 2023 reportee talk to Albert Colli to end their relationship but he disagree d that resulted to
assaulting her by pulling her hair, punching her face, and hitting her head. While A witness known as
Jerson Bites happened to pass by and saw the whole incident and immediately help maria while the
suspect immediately run away after seeing Jerson Bites . Reportee was accompanied by PNP personel of
this office to undergo medical legal examination.

AFFIDAVIT OF WITNESS

I, Jerson Bitez , 25 years old, single , resident of Barangay Tayhi, Tabaco City. after having sworn
in accordance with law, do hereby depose and state THAT:

1. I was present and personally saw that the accused, Albert Colli, committed the crime of serious
physical injury against the victim Maria Bilaro;

2. On May 10, 2023, at about 3:10 in the afternoon, While I was walking to buy some rice, I saw the
suspect Albert Colli and the victim Maria Bilaro arguing with each other, as I pass by Infront of their
house , as the argument continue I heard shouting the word " DAE AKO MATUGOT"

3. There I saw the suspect giving the victim fistic blows, pulling her hair and hitting her head with her fist
and then I came between the two of them and grab Maria after Albert Colli run away.

4. The victim sustained a black eye and bruises all over her face and head, then I immediately brought
her to the nearest clinic

I executed this affidavit to attest the truthfulness of the foregoing facts and to support the filing of
Criminal Cases against Albert Colli, the suspect.
