II. Time, Date & Place Occurred: OOA 8:30 PM of September 19, 2021 along Sitio
Quarry, Brgy. Macabud, Rodriguez, Rizal.

III. Accident Severity : Property Damage

V. Vehicle/s Involved :

Vehicle 1 – SHACMAN DUMP TRUCK with MV file no. 1301-00001333517,

engine no. WP12380E321418H107547, chasis no. LZGJRMT60JX079821, colored white under
the owner’s name PLATINUM REWARD TRADING INC., located at 220H Esguerra St.
Balatong B Pu lilan Bulacan Region 3 and was driven by Luis Rimbao Raposon, of legal age
with address of B153 L21 Brgy. San Isidro, Rodriguez, Rizal (According to Driver’s License)
holder of PDL no. N25-13-003536

VII. Narrative

On the given time and date, driver of vehicle 1 personally appeared to this office
and reported that while he was onboard his driven vehicle and traversing at the place of incident
towards Brgy. Macabud (Proper) when upon reaching a curved portion of the road, suddenly, an
unidentified malefactor onboard a single motorcycle appeared and occupied the lane traversed
by vehicle 1. As a result, driver of vehicle 1, to elude or avoid greater accident or possible loss
of life, driver of vehicle 1 was prompted to a sudden maneuver towards the right side portion of
the road and further resulted to hitting a large branch of tree hanging along the road that
resulted to the vehicle 1 incurring still undetermined amount of damages particularly on the front
right portion, after the incident, the single motorcycle (vehicle 2) immediately fled towards
unknown direction.
