FACT OF THE CASE:

That on or about 8:00 A.M in the morning of May 22, 2022, this station received
a report from the victim, Joel y Bravante, that he was shot outside 1234 Sampaguita
Compound, San Dionisio, Paranaque City. Immediately elements of this station
composed of PCpl. Dexter Montano, PCpl. Ronny Ang, and PCpl. Troy Gallego and
proceeded at the above-mentioned place to conduct investigation.
Investigation conducted revealed that at around 8:30 P.M. in the evening, May
20, 2022, outside 1234 Sampaguita Compound, San Dionisio, Paranaque City, JOEL y
BRAVANTE, Male, Filipino, 23 yrs old, (DOB: March 5, 1999), Single, 4 th Year College
Student, and presently residing at 1234 Sampaguita Compound, San Dionisio,
Paranaque City, was shot by the suspect. And a certain Jimuel y Tiboyac, a friend of
Joel y Bravante, Male, Filipino, 23 years old, born on Feb 01, 1999, Single, 4 th Year
College Student, residing at 1235 Sampaguita Compound, San Dionisio, Paranaque
City.

The aforementioned suspect was angry at Joel y Bravante because Joel and the
ex-girlfriend of the suspect got together after their break up that he shot the victim (Joel
y Bravante) outside the victim’s house. After the shooting, Mark Carvajal and John
Adayon, two witnesses, came out of their home and found the victim who had a
gunshot wound.
Afterwards, Joel y Bravante was immediately took to the hospital for treatment.

Adarlo THROUGH QUESTIONS and ANSWERS.

QUESTION AND ANSWERS

1. QUESTION: MR. JOEL y BRAVANTE, do you swear to tell the whole truth
and nothing but the whole truth in this investigation?

ANSWER: Yes.

2. QUESTION: Are you willing to your free and voluntary statement?

ANSWER: Yes.

3. QUESTION: Will you please state your name and other personal
circumstances?

ANSWER: I’m JOEL y BRAVANTE, Male, Filipino, 23 yrs old, (DOB: March 5,
1999), Single, 4th Year College Student, and presently residing at 1234
Sampaguita Compound, San Dionisio, Paranaque City.

4. QUESTION: Will you please tell me why you are here in our office and in
giving this statement?

ANSWER: To report Jimuel y Tiboyac, a friend of mine, Male, Filipino, 23 years

old, born on Feb 01, 1999, Single, 4 th Year College Student, residing at 1235
Sampaguita Compound, San Dionisio, Paranaque City.

5. QUESTION: What are the reasons and why you’re complaining about the
person you’ve mentioned?

ANSWER: It’s because Jimuel y Tiboyac shot me, and almost got me killed.

 6. QUESTION: When and where the incident took place, if you can recall?

ANSWER: At around 8:30 P.M. in the evening, May 20, 2022, outside 1234
Sampaguita Compound, San Dionisio, Paranaque City.

7. QUESTION: Will you please narrate in this investigation what truly

happened the incident took place?

ANSWER: I got home from a birthday party around 8:25 P.M. when I saw Jimuel
y Tiboyac outside our residence like he is waiting for someone. When I called
him he suddenly took out his gun and shot me.

8. QUESTION: What happened next if there’s anything?

ANSWER: He then ran away quickly and my neighbors Mark Carvajal and John
Adayon who heard the gunshot saw me lying on the floor asking for help. They
immediately took me to the hospital.

9. QUESTION: After the incident, what happened next, if you can recall?

ANSWER: After that, I got treated at the hospital and Jimuel y Tiboyac is
nowhere to be found.

10. QUESTION: Have you ever had an argument or conflict with the person
who stabbed you?

ANSWER: Yes. We are friends. His ex-girlfriend became my girlfriend after 3

months of their break up. I received numerous threat from him that if I don’t break
up with his ex-girlfriend he will kill me.

11. QUESTION: Did someone saw the incident?

ANSWER: Yes. The ones who helped me get to the hospital.

12. QUESTION: In the meantime, I have no more questions to ask, do you have
anything to add, or retract from your statement?

ANSWER: None so far, Sir.

ANSWER: No, Sir. This is my will to give justice to what he had done to me and

JOINT AFFIDAVIT OF ARREST

We, PCpl DEXTER MONTANO, PCpl RONNY ANG, and PCpl TROY GALLEGO, of
legal age, a bonifide member of PHILIPPINE NATIONAL POLICE, and presently
assigned at Ups 5 Police Sub Station/SS2 San Dionisio, Paranaque City, after having
been duly sworn to in accordance with law, do hereby depose and state:

1. That on May 22, 2022, at about 8:00 in the morning, a man named JOEL y
BRAVANTE, Male, Filipino, 23 yrs old, (DOB: March 5, 1999), Single, 4 th Year
College Student, and presently residing at 1234 Sampaguita Compound, San
Dionisio, Paranaque City came to our office at Ups 5 Police Sub Station/SS2,
Paranaque City Police Station to report the shooting incident that happened
outside 1234 Sampaguita Compound, San Dionisio, Paranaque City.

2. That we immediately went to the said place of incident to verify the said
information/report we received. When we arrived at the said place, we asked the
neighbors named Mark Carvajal and John Adayon that helped Joel y Bravante
rushed to the hospital to confirm the said shooting.

3. That we, PCpl Dexter Montano, PCpl Ronny Ang, and PCpl Troy Gallego,
immediately went to the suspect’s house and arrested a man named Jimuel y
Tiboyac, Male, Filipino, 23 years old, born on Feb 01, 1999, Single, 4 th Year
College Student, residing at 1235 Sampaguita Compound, San Dionisio,
Paranaque City.

4. That we, PCpl Dexter Montano, PCpl Ronny Ang, and PCpl Troy Gallego,
identified ourselves to the suspect as police officers and members of Ups 5
Police Sub Station.
5. That Jimuel y Tiboyac peacefully surrendered himself to us and also,
surrendered his gun that he used on the said incident. I PCpl Dexter Montano
took the said caliber with care so that it won’t be fingerprinted by ours. 38-pistol
caliber that has four (4) pieces of Live Ammunitions and one (1) empty Cartridge
Shell, while PCpl Ronny Ang and PCpl Troy Gallego was informing the reason of
his arrest and of his constitutional rights and all those rights were explained to
him in a language that he understood;

 6. That we, immediately brought the suspect to our office at Ups 5 Police Sub
Station/SS2, Paranaque City Police Station along with the evidence that we
obtained from the suspect.

7. That after, the said suspect was given medical attention at the nearest hospital. I
PCpl Dexter Montano also marked the 38-pistol caliber PBA 4-8-22, four (4) live
ammunitions are PBA 4-8-22-1, PBA 4-8-22-2, PBA 4-8-22-3 and PBA 4-8-22-4
while the other one (1) empty Cartridge Shell is PBA 4-8-22-5. After making the
relevant documents for turning over, we immediately went to our office at Ups 5
Police Sub Station/SS2, Paranaque City Police Station to file a case against the
suspect. And we immediately went to the SOCO to turn over the evidence
(38pistol caliber) for ballistic test.

8. That I executed this affidavit/statement to attest to the veracity of the

aforementioned facts and support the filing of appropriate charge/s in court
against the arrested suspect.
