INCIDENT ASSESSMENT
DAMAGE Transformer oil loss (minor); Minor oil spill at transformer area
ASSESSMENT
CAUSE OF INCIDENT Oil leak from radiator pressure gage fitting (in droplets)
RESPONSIBLE Transformer Supplier; The equipment is not yet energized & was only installed at site by JTDI; No
PERSON/PARTY modifications/alterations were performed on the transformer parts, especially on the leaking pressure gage, &
there were no activities on the area before & during the incident.
DAMAGE REMEDY Removal (if necessary), check-up (oil seal), replacement (if necessary), & proper tightening of reported
pressure gage fitting (coupling reducer); Clear up/clean oil spill
INCIDENT DETAILS:
1) On April 4, 2023, Mr. Shuqi Wen of JTDI reported to NHVS engineer at site, Rey Pamelo O. Navarro, that the
SSTR transformer at the new auxiliary bldg. is leaking. NHVS engineer inspected the transformer and found
the oil leaking from the pressure gage fitting installed at the radiator manifold/header pipe (top). Presence of oil
spill on the transformer pad was also found as a result of the leak. The transformer is not yet energized but is
exposed to direct sunlight. JTDI installed a small bucket to collect the oil leak and prevent the oil spilling on the
transformer pad. No ongoing activities found on the transformer area.
