BASIC LIFE SUPPORT CHECKLIST

BLS Participants for Healthcare Providers/ Lay Rescuers

1. 13 yrs. old – 59 yrs old

2. High risk and vulnerable individual (elderly > 60 years, immunocompromised, or those with serious or
unstable co morbid conditions) are not allowed.
3. Not pregnant
4. Valid Medical Certificate (must present individually/ will be collected prior the start of training) with
the following requirements:
● Physically fit for the training
● No respiratory symptoms at least 10 days
● Not a COVID-19 suspect or close contact with a confirmed case
5. Must wear Protective equipment on the training (Face Mask)
6. Maximum of 20 participants per training.
7. Venue: must be well-ventilated open space. Enough to perform skills enhancement at least 1 meter
for each pax. Seating arrangement must be at least 1 meter apart.
8. Food – Packed meals service with bottled water or beverages.

BLS Participant’s Kit [INDIVIDUAL]

1 kit per participant and must contain the following:
● Brown or Plastic Envelope (Long) [Kamo la bahala]
● 1 BLS Workbook
● 1 Participants Record (Filled-out legibly)
● 1 Skill Sheet (reproduced back-to-back) Adult CPR and Infant CPR
● 1 Course Evaluation Sheet
● Facilitators Evaluation Sheet (quantity depends on the number of faci)
● 1 Answer Sheet (Lengthwise)
● Extra Paper for notes
● Ball pen or Pencil
● 3 pairs of clean/sterile gloves
Note: Each form must have extra copies

Materials and Supplies [WHOLE GROUP]

● Specialty Board Paper White (3 packs)/ Letter size

● Specialty Paper (Unscented) Cream White (6 packs)/ Letter size
● 2 Cotton Balls (Big Size) / Alcohol 500ml
● 4 Alcohol sprayer [Bisan empty, ako na madara hin alcohol]

Equipment

● LCD Projector
● Printer
● Ink colored/ Black (Extra)
● Laptop (1 preferably)
● *Questionnaire/ Examination Paper – c/o DOH
● Bag Valve Masks (Ambu Bag) – 7 infant / 7 adult c/o DOH
● Mannequins [1 adult, infant] – c/o DOH
● Automated External Defibrillator (AED) – c/o DOH
